To help meet the demand for local diabetes services, our patients can now receive diabetes self-management education and support right within their Primary Care Provider’s office.

This evidence-based, accredited service helps people manage diabetes through personalized plans, including nutrition, exercise, medication management and glucose monitoring.

“The goal is to educate people on how to manage their diabetes to stay healthy, avoid preventable complications and keep people out of the hospital,” said Jennifer Putzel, our Manager of Ambulatory Care Management.

Our pharmacies have gone green—literally! ♻️

Our prescriptions are now packaged in compostable, paper-based and curbside-recyclable prescription bottle Tully Tubes from Parcel Health. To date, Parcel Health has prevented over 250,000 plastic bottles from entering the waste stream, equivelant to about five school buses full of plastic.

It's our last week collecting donations for Monadnock Center for Violence Prevention, supporting the rest, comfort and healing of survivors who are staying at their emergency shelter.

⚠️ Maple Avenue detour this week ⚠️

On Monday, April 27, from 7 am to 7 pm, a portion of Maple Avenue between Court Street and Pako Avenue will be closed for the City of Keene to safely harvest red pine trees

On Tuesday, April 28, through Friday, May 1, Maple Avenue between Melody Lane and Court Street will be closed from 7 am to 7 pm.

These closures will not affect the entrance or exit to our West Campus (Family & Community Care); however, patients with appointments should be prepared to use marked detours and expect traffic delays.

Join us this Saturday, April 25, from 9 am to 1:30 pm for our weather-permitting Touch-a-Truck event! 🚑

Children and adults of all ages will be able to explore the Dartmouth Hitchcock Advanced Response Team (DHART) aircraft and ambulance, a West Station ambulance, a West Station fire truck and a N.H. State Police Troop C cruiser and BEAR.
