Beat the school rush and request your child’s MS121 Immunization form online today!

Do I need a Mississippi Form 121 every year? YES, if:
Entering kindergarten for the first time
Entering a Mississippi school for the first time
Entering a new school district
Entering 7th grade for the first time

Do I need a Mississippi Form 121 every year? NO!
If your child is returning to the same school they attended last year, the school has it on file. Exception: entering 7th graders.

My child is current on vaccines, but I need the MS 121 form:
Request a signed form through Keesler’s Immunization’s Genesis Patient Messaging Portal or schedule in-person appointment for an updated form.

My child needs vaccines and an updated form: Schedule in-person appointment for vaccination and 121 form update by calling the appointment line (228) 376-2273.
Locate Immunization’s Message Pool on Genesis Patient Portal: patientportal.mhsgenesis.health.mil Click “Send a Message” on the home page and select “Keesler Immunizations Clinic” in “To” box. Type message and send. The request must come attached to the child’s patient portal account, so we have the correct immunization records, name, and date of birth.
My child gets vaccines in MS but not at DoD facility: Retrieve your children's immunization records online without a health visit. Register (free) with the MyIR system at https://myirmobile.com/ to get started. Need help? Check out https://msdh.ms.gov/MyIR or call 1-877-978-6453.
