ROLE AND RESPONSIBILITIES

Ensure quality and safe delivery of home care services.

Participates in development and implementation of client plans of care per home care agency policy and procedure, as appropriate.
Participates in client case conferences according to home care agency policy and procedure, if requested.
Information regarding client plans of care is submitted to the Home Care SUPERVISOR in a timely manner.
2. Implements current Certified Nurse Aide services.

Service plans of care are discussed with the Akoma Home Care’s Supervisor on regular basis.
Client assignments and reports are received from the Akoma Home Care’s Supervisor.
QUALIFICATIONS AND EDUCATION REQUIREMENTS-High school graduation or GED required.-Certified Nurse Aide certification required as obtained through successful completion of an approved program-Shall have one year-full-time experience in personal care or an institutional setting, such as a hospital or nursing home, OR Shall have one-year full-time experience within the last five years in direct client care in a home care agency setting; OR-Evidence of sympathetic attitude toward care elderly.-Demonstrated ability to read, write, and carry out directions.-Evidence of maturity and ability to deal effectively with job demands-Good verbal and written communications skills required.

-Attends (8) hours of personal care oriented in services per year.
-Participates in professional meetings when directed
-Shall have a criminal history check conducted prior to being offered work with this agency.
-Ability to work closely supervised to ensure competence in providing client care.
PHYSICAL DEMANDS
-Must be able to read & write English.
-Must be physically able to lift, bending and stand for long periods of time.
-Must be able to handle stressful situations due to client’s medical conditions.
-Must adhere to assigned and agreed work schedule.
-Able to drive up to 30 + miles per day.
-Must maintain professional appearance and good personal hygiene.
COMPETENCY TESTING:
Certified Nurse Aide is expected to pass competency examination with at least an 80% or better. The content of the competency evaluation of the Agency will include and is not limited to: (if applicable)

o Strong sense of initiative and urgency in addressing unexpected patient crises

o Self-directed, able to work without presence of immediate supervisor.

o Commitment to regulatory compliance and to submitting all documentation in a timely fashion

o Demonstrated knowledge regarding exposure to body fluids and chemicals, as well as to some hazardous equipment

Must have the ability to:

o See and hear adequately to respond to auditory and visual requests.

o Pass a background screen.

o Travel as required and be in possession of a valid driver’s license and automobile insurance.

Responsibilities:

o Develop and implement the service plan as applicable and regularly reassesses the needs of the client.

o Participate in in-service programs, as well as training, and teaching other personnel.

o Develop and implement the service plan for each patient under the direction of their physician or family.

o Observe and report symptoms, reaction to treatments, drugs, and changes in the patient’s physical or emotional condition.

o Initiate preventative procedures as appropriate for the patient’s care and safety and effecting changes in environment to ensure patient safety and security.

o Maintain clinical and progress notes for each patient receiving care and providing progress reports to their physician.

o Coordinate services for patients as needed to other agencies and health care providers as appropriate.

o Counsel the patient and their family in meeting nursing and related needs.

o Provide supervision of caregiver services and preparing written instructions for care provided by aides.

o Maintain confidentiality of patient and agency matters.

o Submit required documentation in a timely manner.

o Report observed or suspected abuse pursuant to mandated requirements.

o Assist the Administrator in decision making and policy formation as it pertains to the Home Care Services.

o Participate in surveys, studies and special projects as assigned.

o Maintain accurate time records

o In addition to scheduled hours, participating in weekend/holiday call schedule, as needed.

o Observe agency’s attendance requirements and dress code.

o Conserve agency resources and adequately maintaining agency property, supplies, and equipment.

o Adhere to all agency polices, including but not limited to, infection control and safety, education, reporting and practice implementation.

Pay: $40.00 - $50.00 per hour
