Transition Rehab provides supportive housing and intensive therapeutic services for adult women living with mental, psychological, and emotional disorders and who are seeking to live a more meaningful and productive life. We coach each woman individually, providing them with full case coordination, life skills coaching, support services, respite rehabilitation and community resource guidance. Transition Rehab also offers various Respite programs, and Therapeutic Groups and Workshops for family members and caretakers. Janice A. Shomo, LMSW, is the Founder & Owner of Transition Rehab, and is a Licensed and insured Social Worker, a Certified Addiction Specialist and Notary who has been actively working in the fields of behavioral/mental health, addictions, criminal justice and communicable diseases for twenty years.
