After a fall and hip fracture, Laurie came to Powerback Rehab at Brookdale Senior Living in South Windsor, CT determined to regain her independence and get back to the outings she loves. 💪

With perseverance and partnership with our team, she made incredible progress, going from needing maximum assistance to transferring on and off the community bus with just light support. Her standing tolerance also grew from just 60 seconds to a full 10 minutes!

Laurie’s story is a powerful reminder of what’s possible with dedication, the right support, and a little Powerback spirit. 🌟

At Saint John Paul II in Danbury, CT, our “Gentlemen’s Home Management Group” is more than therapy, it’s a space for connection, confidence, and independence.

Led by Claire Lumbrera, OTR, this group therapy program focuses on helping male residents who live alone regain independence in daily living and home management skills. What started with quiet, reserved participants quickly grew into an encouraging, supportive community. By the end of their sessions, these gentlemen were not only sharing strategies and solutions but also cheering each other on every step of the way.

CMS will soon begin reviewing MDS submissions to ensure accuracy in reporting for SNF Quality Reporting and Value-Based Purchasing Programs.

👉 What to know:
✔ Facilities meeting eligibility may be randomly selected
✔ Notifications will be posted in iQIES under the MDS 3.0 Provider Preview Reports folder
✔ Selected SNFs must submit supporting documentation for up to 10 MDS assessments within 45 days

Accurate and timely documentation is key to staying compliant and supporting quality care outcomes.
