Not all under-eye darkness can be improved with filler.

Filler can be a powerful treatment when indicated and performed judiciously, but I see, time and again, patients are treated with filler when their presentation requires an alternative modality.

The result? A subpar outcome.

The patient above presented with a not uncommon anatomic variation called “A-frame deformity.” This is when a hollow appearance in the upper eyelid creates a visible “A” or “V” shape near the inner corner of the eye.
This shadowing can cause the eyes to appear gaunt and sunken.
