Care Resources PACE

Care Resources  PACE For most people, there are little to no deductibles or co-pays. Participants who do not meet PACE eligibility guidelines can pay for services privately.

Care Resources is a Program of All-inclusive Care for the Elderly, or PACE®, that provides wrap-around supportive services for people 55 years and older to keep them healthy and living independently in their homes. We partner with participants and their loved ones to develop a comprehensive care plan that meets the physical, mental, emotional and social needs of each participant and keeps them out

of nursing homes. Through our day centers in Grand Rapids and Lowell, we offer:
• Primary health care
• Home health care
• Clinic
• Prescription medication
• Rehabilitation services
• Activities
• Nutrition services
• Social services
• Transportation

PACE uses Medicare and Medicaid funds to cover all medically necessary care and services. Care Resources PACE serves those who are 55 and older and are:
• Residents of Kent County
• Residents of Allegan, Barry, Ionia and Ottawa counties living in these ZIP codes: 48809, 48815, 48846, 48849, 48865, 48881, 48897, 49058, 49302, 49315, 49316, 49323, 49325, 49328, 49331, 49333, 49344, 49348, 49418, 49428, 49435, 49534, 49544

We’re proud to be a gold sponsor of the Lowell Riverwalk 5K, one of many fun events planned for the 30th Annual Riverwalk Festival!

It’s honor to give back to a community that has embraced us since opening our Lowell center at the beginning of this year! Walk or Run the Riverwalk 5K with us Saturday, July 12. Find all event details at DiscoverLowell.org/run-the-riverwalk-5k.

All proceeds raised will support a community of widows and orphans in Swaziland, Africa. The funds will provide assistance for food, school fees and building projects.

Universal Worker Ron "RJ" Eubanks is our June RISE recipient!

Here’s what our “Recognize, Inspire, Shine, Exceed the Limits” Committee had to say about Ron:

Ron is the ultimate team player and understands we all have the same goal. He is always stepping into any role to ensure participants are taken care of. He is a universal worker, but he does not need to be asked to assist. He always makes himself available to participants as well as staff.

His attitude is, “whatever I can do to help, I will.” He is often there and available before we even have the chance to ask him. He jumps around to each department and sees where he can be the most helpful, especially in the kitchen and in the day rooms. He helps with daily meal prep and with serving meals in the day center. Additionally, he brings participants home in the van, one of them every Thursday afternoon. He also assists daily with unloading and reloading the buses. He even covers the front desk while the receptionist is at lunch.

It’s not just all the extras RJ does, it’s his supportive and positive attitude he brings to work every day. RJ, you’re a shining example of why Care Resources is so special. You’re a hardworking, flexible professional who really cares about our participants and is happy to help out with whatever the need may be. Thank you and congratulations! 🎉🎉
