One on one sessions allow us to establish a personal, comprehensive treatment plan to help you improve your quality of life.
Commonly Treated Diagnoses include but are not limited to: Post Surgical Rehabilitation,Neck & Spine Injuries, Muscular Strains, Extremity Injuries, Fibromyalgia, Low Back and Hip Pain, Post Fracture/Immobilization, Arthritis/Osteoporosis, Cervical Pain, Tendinitis, Vertigo and Nerve Impingement
Appointments can be scheduled by phone at 808-661-5266. Please have your doctor fax a referral to our office at (808) 661-5264. We will also need to make a copy of your drivers license and health insurance information at your first visit. Our Patient Information/Medical History Form can be downloaded from our website or can be completed at the office.
An Initial Evaluation will be conducted to determine the range of motion, strength, endurance and changes in neurological function. Your therapist will address any questions or concerns and begin treatment that will allow you to take control of your physical health. The number of visits will depend on the recommendations between you, your physical therapist and your physician.
Lahaina Physical Therapy participates with many health insurance providers including HMSA, Kaiser, HEMIC, VA, HMAA, Humana, Sedgewick, Farmers Insurance, UHA, Tricare and Medicare to name a few. In order to have your health insurance provider cover the costs of physical therapy (coverage amount depends on your individual health plan), you are required to have a referral from your physician. Hawaii is a direct access state, therefore if you would like to pay privately, you do not need a referral.
Dickenson Square is located in the heart of Lahaina, on the corner of Wainee St and Dickenson St. Our office is on the second floor above Lahaina Coolers with elevator and stair access. Parking is free for the first hour.
