Dear PhyNet Kilgore Patients

To better coordinate care services within our integrated network, PhyNet will be transitioning clinical services from the Kilgore Clinic to other PhyNet Clinics effective November 1, 2025.

This transition supports our mission to strengthen access to better coordinate care across our region-leading network of Primary Care, Home Health, Hospice, and Outpatient Physical Therapy services.

Your medical records will be transferred to PhyNet Hugman-Kent Clinic or PhyNet Tatum Medical Clinic based on your home address to ensure continuity of care. Watch your mail for updates. However, all eight PhyNet clinics across East Texas share a unified electronic health record system, so you are welcome to receive care at any location that is most convenient for you.

Medical Assistant (MA) – Care Coordination
(Not Remote–Working with Virtual CareTeam in Longview, TX)

Join Our Team! PhyNet is seeking an MA to work with the care team from a centralized office setting, focusing on remote patient support and care coordination rather than in-person clinical tasks. The role helps ensure ongoing engagement for patients with chronic conditions.

CORE ROLES:

Perform follow-up calls for routine check-ins, medication adherence, and care plan compliance.

Coordinate appointments, referrals, and diagnostic services.

Document interactions and care updates in the EHR.

Assist with tracking preventive care and managing care gap lists.

Communicate with patients and providers to support continuity of care.

CERTIFICATION / EXPERIENCE:

Certified MA preferred; BLS

Prior experience in remote patient coordination or outpatient care setting is a plus.

Keep in mind: Caller IDs and email addresses can be faked, so if you're not sure who's calling, call us at 1-800-MEDICARE (1-800-633-4227). And remember, Medicare will never contact you unless you've contacted us first.

It's important to know the signs of a Medicare scam so you can protect yourself. To get your personal information, like your Medicare number, scammers may call, text and email saying things like:

"You need to activate or renew your Medicare card."

"We'd like to send you a free genetic testing kit."

"Medicare has authorized us to send you a free (knee, back or elbow) brace."

"You qualify for a refund on your Medicare costs."
Remember, never give your Medicare card or number to anyone except your doctor or people you know should have it.
