Unfortunately, our power is currently out. We are so sorry for the inconvenience but we cannot see any urgent care patients at this time. We are unsure when it might come back on but will post as soon as it does!

Family 1st Urgent Care is excited to announce that we are now accepting Primary Care Patient Applications! 🩺

If you are interested in becoming a primary care patient with us, please complete an application at the link below:

Important Information:
• Applications must be printed by the patient (you may use a local library if needed). We are unable to print them in-office.
• Completed applications can be mailed or dropped off — we are not able to accept applications by email.
• While we are accepting applications, all submissions will be reviewed by our providers prior to approval.
• If approved, you will be contacted at the end of May.
• Please do not request medical records until you have been approved by our providers.
• We are planning to open Primary Care services in early June.
• We are currently working on insurance credentialing. The application will ask for your insurance information, and we will contact you if we are unable to accept your plan.
• Applicants must be 18 years or older.
• If you are already an established primary care patient, please complete a new application and note that on your form.
