Important Update – Office Closure and Transition of Care
Dear Patients,
I am writing to share an important update regarding my medical practice and my personal health. As many of you know, I recently experienced a medical emergency that required a series of surgeries, followed by ongoing treatment. While my recovery is progressing and I am currently in a specialized rehabilitation center, the road ahead involves several months of intensive physical therapy and significant lifestyle adjustments which will take substantially longer.
Due to the extended nature of this recovery period and my age generally, I have made the difficult decision to retire early and close the practice. This decision was made with a heavy heart but one that I believe is necessary to ensure that you continue to receive the consistent, high-quality care you deserve from a provider who can be fully present.
Key Dates for Your Care:
• Last Day of Clinical Appointments: March 13, 2026
• Official Office Closure: March 27, 2026
Between now and March 13th, our temporary physician, Dr. Selig, will continue to provide in-person well and sick visits.
During this time through March 27, 2026, our office staff will also continue to be available to assist with the administrative transition and the transfer of medical records.
Next Steps for Your Continued Care:
To facilitate your transition to a new medical provider, please consider the following:
• Medical Records: You may request a copy of your medical records until July 1 2026 through our patient portal or authorize a transfer to a new provider by contacting our office prior to March 27. We strongly suggest reaching out as early as possible in order to provide sufficient time to process those requests. Following that date, patients will be able to access their medical records through an electronic medical records custodian which will be set up in accordance with applicable medical requirements. Our office will send further details regarding this custodian prior to office close.
• Prescriptions: Please ensure that you have sufficient refills to last through this transition. Our temporary physician, Dr. Selig, can assist with urgent prescription needs until March 13th.
• Finding a New Provider: If you need a referral to another provider, we recommend reaching out to your insurance carrier. They can help you identify providers who are covered under your plan and best suited to your needs.
From a personal note, serving as your physician has been a profound privilege. I have deeply valued the trust you placed in me and the opportunity to care for you and your families, to watch children grow up, and to be part of so many important moments in our community.
Practicing medicine here has truly been more than a career — it has been a calling and a joy. Thank you again for allowing me to be part of your lives.
If you would like to stay in touch, please note that I will be moving permanently to Cape Cod and would be delighted to hear from you. Please contact the office for my updated contact information as it becomes available.
Warmest regards,
Dr. Azra Sehic
