NOW HIRING: COMMUNITY LIAISON – EDENBROOK SECOND AVENUE
Edenbrook Second Avenue is seeking an outgoing, relationship-driven professional to join our team as a Community Liaison. This role is ideal for someone who loves connecting with people, being out in the community, and helping families and providers understand the care we offer.

WHAT THIS ROLE INVOLVES:
• Building strong relationships with hospitals, rehab centers, and community referral partners
• Promoting Edenbrook Second Avenue’s rehab programs, outcomes, and patient-centered care
• Attending community events, healthcare meetings, and networking opportunities
• Hosting tours for families, case managers, and referral partners
• Tracking referral trends, identifying new opportunities, and supporting census growth
• Supporting the admissions team with communication and pre-admission information

REQUIREMENTS:
• Experience in healthcare marketing, admissions, outreach, or sales
• Strong communication and relationship‑building skills
• Valid driver’s license and reliable transportation
• Knowledge of skilled nursing or post‑acute care preferred
