Clean notes don’t always mean accurate documentation.

Templates make documentation faster… easier… more consistent.
But when they start running on autopilot, they stop supporting the encounter, and start replacing it.

And that’s where the risk shows up.

Because when every note looks the same, auditors start asking questions.
When language repeats, credibility gets tested.
When documentation doesn’t match the patient, it doesn’t hold up.

At the end of the day, documentation isn’t judged by how complete it looks,
it’s judged by how well it reflects what actually happened.

If it could belong to any patient… it defends none.
