A scribe may document the encounter, but the provider is still responsible for the note.

One of the biggest compliance mistakes we see is relying on a signature alone and forgetting the attestation. If the documentation does not clearly show provider review and involvement, it can create major audit risk.

A strong scribing attestation does not have to be long or complicated. It just needs to clearly connect the provider to the service documented.
