I can now share with everyone what Dr. Jonathan Downar told us at the Fall Psychiatric Symposium. They just published a pre-print article on a 1 day accelerated TMS protocol using intermittent theta burst, 20 sessions over 9.5 hours (2/hour) with an astonishing 92.3% response and 73.1% remission rate on the BDI-II! They used 2 mediations, d-cycloserine and Vyvanse 20mg prior to the treatment to prime neuroplasticity. The article can be found at https://www.researchsquare.com/article/rs-5679327/v1

A Dutch study just published September, 2024 in the American Journal of Psychiatry. In a randomized control trial comparing persons with severe depression who failed 2 antidepressants between 25 sessions of left-sided DLPFC vs a switch to another antidepressant. TMS resulted in a significantly larger reduction in depressive symptoms than medication, which was also reflected in higher response (37.5% vs. 14.6%) and remission (27.1% vs. 4.9%) rates. A larger decrease in symptoms of anxiety and anhedonia was observed after rTMS compared with a switch in antidepressants.

In a sample of patients with moderately treatment-resistant depression, rTMS was more effective in reducing depressive symptoms than a switch of antidepressant medication. In addition, the findings suggest that the choice of treatment may be guided by specific symptom dimensions.

So I am lecturing again at the Fall Psychiatric Symposium (I think it's my 6th lecture) and excited to be once again bringing in Jonathan Downar, M.D., Ph.D. from Toronto, world famous TMS researcher who will be speaking on TMS in 2024: New Techniques, New Protocols, New Indications. This is the southeast's largest psychiatric CME symposium. Hope to see some therapists and new PMHNP's and PA's at my lecture. TMS of Knoxville and MagVenture will be exhibiting as well.

What if you don't respond/remit by 36 sessions? Here's a graph from an article in Brain Stimulation Journal about patients who did not respond until the 10th week. We give 10 extra sessions to people we consider may be late responders. We bill the insurance company if they precertify more sessions or we give them for free!
