Getting started with TRU PACE is easier than many families expect. Here’s how the enrollment process works:

Step 1 – Initial Contact
A participant, family member, or healthcare provider contacts TRU PACE. Our Intake Specialist gathers basic information and discusses care needs and eligibility.

Step 2 – Enrollment Visit
An Enrollment Specialist reviews the program, answers questions, and discusses medical history, insurance, and support needs.

Step 3 – Health Assessment
Our Interdisciplinary Team evaluates medical and care needs to determine eligibility for the program.

Step 4 – Welcome to TRU PACE
Once approved, enrollment is completed and services begin.

Participants can also choose to disenroll at any time.
