SouthernCare is a hospice company that provides palliative care to terminal patients wherever they reside. At SouthernCare we have the opportunity and thus the obligation to listen, interpret, and then provide the best way to improve the quality of life of our customers by exceeding their expectations and fulfilling their wishes one person at a time. The Lafayette office is managed by the Clinical Director, Joanna Turner, RN and Community Relations Director, Jon Hooyenga. Also on staff is a dedicated Care Team composed of Registered Nurse Case Managers, Social Workers, Hospice Aides, a Chaplain, and a Volunteer Coordinator. Trained Volunteers are also a valuable support to our staff an dour patients. Hospice is a Medicare Benefit and can also be covered by Medicaid and some private insurances.
