shes one person at a time. The Lafayette office is managed by the Clinical Director, Tammy Dodt, RN and Community Relations Director, Jon Hooyenga. Also on staff is a dedicated Care Team composed of Registered Nurse Case Managers, LPN Case Coordinator, Social Workers, Hospice Aides, a Chaplain, and a Volunteer Coordinator. Trained Volunteers are also a valuable support to our staff and our patients. Hospice is a Medicare Benefit and can also be covered by Medicaid and some private insurances.
