19 yo female patient with a 7 year hx of lateral lower leg pain within a quarter mile of running and arch pain. She had a bag full of OTC and one custom pair of orthotics and failed PT x2.

What no one had assessed properly was a large partially compensated forefoot valgus, one of the larger ones I have observed in recent memory.

A colleague honored me by flying out from AZ to have his foot pain evaluated. I'll let the picture of the "custom" insole he is wearing and the pop cast of his foot do the talking.

Does that arch even vaguely approximate his naturally high MLA? Nope.

We recently had a casting workshop to demonstrate foam impressions and plaster casting technique. We also castes two doctors full weight bearing vs semi weight bearing and the results are below. It all begins with a volumetric, off weight bearing or semi weight bearing cast, the results below speak for themselves.
