Mrs. Diane chose Carriage House Manor following a left femur fracture and continued her journey with Part B therapy after her skilled stay.

With determination, support from her husband, and guidance from our therapy team, she worked hard to regain her strength and confidence, overcoming not only physical challenges, but also a fear of falling.

Today, she is making incredible progress toward her goal of returning home.

We are so proud of Mrs. Diane and the dedication of Rome, PT, for walking alongside her every step of the way.

At Synergy Care, every journey matters.

Ms. Catherine chose Maison du Monde Living Center with the goal of receiving skilled therapy services to regain her strength and independence. She began with simple seated movements and required significant assistance for transfers.

Through consistent, personalized therapy, her strength and balance improved, progressing from maximal assistance to independence with wheelchair transfers.

We’re proud to share that Ms. Catherine was able to safely return home and resume her daily routines.

Julie, Ethan, and Nicole had a great time at the Texas Assisted Living Association Conference and Industry Expo!

Julie and Ethan enjoyed connecting with current partners and meeting new communities, sharing more about the Synergy Care difference, how our therapy services, clinical programs, education, and partnership approach come together to truly elevate care and outcomes.

Nicole had the privilege of presenting on advancing person-centered dementia care. These conversations matter, because how we care for individuals living with dementia shapes not only outcomes, but dignity, connection, quality of life, and joy. It was encouraging to see these ideas resonate and spark meaningful engagement in the room.
