Lake City Care Center provides both long- and short-term transitional skilled nursing care and holds a Centers for Medicare & Medicaid Services (CMS).

We were deemed a 5-star facility in 2021. We were also named US News & World report as Best Nursing Home and Short Term Rehabilitation in 2021-2022. As part of the Ebenezer family of Senior Care communities, we are dedicated to helping older adults and others make their lives more independent, healthful, meaningful, and secure. You have an essential role in providing the highest quality of care to residents and their families. Our industry is never short of challenges, but your empathy, skill, and dedication to our communities and to the practice of nursing make a positive impact on all the lives around you.

It is my tremendous privilege to express my heartfelt thanks for all you do to make a difference every day. You put residents first by advocating for them, understanding them and treating each of them as an individual with the utmost compassion and care, often during some of the most challenging times in their lives.
