📢 Important Message for Our Patients with UnitedHealthcare (UHC)

We want to assure all our valued patients that there is no need to be alarmed if you recently received a letter from UnitedHealthcare stating that our office will no longer be in-network.

This letter was sent in error — we have confirmed that our office will remain in-network with UHC, and you can continue seeing your provider as usual.

We sincerely apologize for any confusion this may have caused and have appreciated your patience as this issue was being corrected by UHC.

Clarification Regarding Office Status:

We would like to clarify that Dr. Patel’s office remains open and fully operational. We understand there may have been some confusion following Dr. Choudhury’s recent retirement announcement in the Lake City Reporter.

We are pleased to share that Dr. Patel’s office will be welcoming Dr. Choudhury’s patients and will continue to provide the highest standard of care and support.

Dr. Choudhury’s office will be closing as he begins a well-deserved retirement. We extend our heartfelt congratulations to Dr. Choudhury and wish him all the best in this new chapter.
