The patient is snoring. Phase 1 orthodontics failure.

Not because the maxilla wasn’t expanded — that part was done.

The problem is the mandible is trapped in retrusion.

When the lower arch is narrow and uncoordinated with the expanded maxilla, the mandible cannot seat forward physiologically. It is forced posteriorly by occlusal interferences.

That posterior positioning reduces the retrolingual airway space.

The tongue follows the mandible.
The hyoid follows the tongue.
The airway narrows.

Now we see:
• Snoring
• Mouth breathing
• Increased airway resistance
• Potential sleep-disordered breathing

This is not just a bite issue.
This is a functional airway issue.

If the mandible is trapped back, the airway is compromised.

Orthodontics is not just alignment.
It is structural support for breathing. The maxilla was expanded skeletally — and that part was done correctly.

But the lower arch was never addressed.

When you expand the maxilla orthopedically and leave a constricted or lingually tipped mandibular arch untreated, you create a transverse mismatch.

Now the patient presents with:
• Traumatic occlusion
• Occlusal interferences
• A functionally trapped mandible
