Safe Transitions Home (STH) is a physician-led program designed to provide HIPAA-compliant, post-discharge care through advanced telehealth solutions. Our goal is to reduce preventable emergency department visits and hospital readmissions by focusing on early intervention and timely identification of patient needs.

Each patient receives a telehealth appointment within 24 to 72 hours post-discharge, ensuring continuity of care and proactive clinical oversight. STH works closely with community partners to connect patients with the right provider, at the right time.
