Video of Septal Space Filling for Tip Elevation

Dr. Arthur Yu authored an article in Aesthetic Surgery Journal on a safe noval technique for nasal top elevation. I attended his lecture at the last VCS conference in Las Vegas. This technique involves injection into the septal space between the anterior edge of the septal cartilage and the medial crura.

Once you each the nasal spine, about 0.4~0.8 of filler is deposited up to the mid level of the medial crura. This is a safe space to deposit filler as it relatively avascular compared to neighboring areas. Less change of vascular compromise with compression or intravascular embolization. Another advantage with this technique compared to the conventional layered technique of placing filler on the cartridges or in the subdermal area is prevention of a bulbous nose. If you look at the patient from the bottom of the nose up, you notice a more narrow nose with a normal lobule and longer columella.

This technique worked nicely on this patient who desired a tip elevation .

This patient was looking for lift in the lower face and neck. Dr. Quita Lopez used a newer technique to create a sling with a single thread — keeping the cannula in place while redirecting the thread into a new vector and advancing it until release.
