This position carries a routine schedule of Thurs, Fri, and Sat from 7am-7pm.
The Program Nurse is responsible to meet the basic medical needs of the client under the direction and supervision of the Medical Director. Contributes towards the planning, delivery, and evaluation of client care.
Monitor, record and report symptoms and changes in patients' conditions.
Record patients' medical information and vital signs.
Consult and coordinate with health care team members to assess, plan, implement and evaluate patient care plans.
Order, interpret, and evaluate diagnostic tests to identify and assess patient's conditions.
Prepare patients for, and assist with, examinations and treatments.
Instruct individuals on topics such as health education, disease prevention and medication side effects.
Prepare rooms, sterile instruments, equipment and supplies, and ensure that stock of medical supplies is maintained.
Provide health care, first aid, immunizations and assistance in the treatment setting.
Conduct specified laboratory tests.
Responsible for keeping medication observation/dispensing operations streamlined and efficient.
Reconcile pharmacy orders daily to ensure all medication that is ordered is received and any medication not ordered is returned.
Works collaboratively and effectively with other staff and professionals. This will include coordination and participation with administration, support staff, clinicians, case managers and other medical colleagues.
Complies with agency policies and procedures including those pertaining to medical services and medication management
Maintain a customer-oriented approach to meeting patient needs in an expedient manner, involving the family in the plan of care as appropriate.
Meet with new clients and assist in completing the medical and health assessment/ HIV/TB/AIDS Risk assessment within 24 hours of admission.
Participates in scheduled meetings, and supervisory sessions.
Ensures confidentiality is maintained at all times and in accordance with agency standards
Adheres to agency Exposure Control, Risk, and Safety Management Programs and participate in the management of such as requested by leadership
Ensures the maintenance of the Medication Administration Records for each client, and revises as necessary.
Ordering all medication prescribed by Medical Director, and/or Nurse Practitioner through local or vending pharmacy and ensuring all medication is received in a timely manner.
Identifying documentation that must be reviewed by the Medical Director, and/or Nurse Practitioner and ensuring that a signature confirming review is indicated on documentation.
Ordering medical supplies for the organization as needed.
Provide medically related group therapy sessions as assigned by administration.
Performs Vital Signs as appropriate and necessary to assess medical stability.
Performs basic first aid and collection of laboratory specimens.
Seek supervision and direction from Medical Director, Nurse Practitioner and/or Staff Physician when observation of client’s medical status renders medical intervention beyond scope of practice or expertise.
Performs other duties as required.
