While we're not a medical practice, and I typically describe our services as more akin to than , our clients are dealing with too many medical issues for us to not be heavily involved in their medical care. It's why I'm so glad to have an and an on board!
π« We accompany clients to their medical appointments to ensure all concerns are brought to the physician's attention. We track symptoms and complaints to give the physician a more comprehensive picture of their patient, which is particularly important for clients with cognitive decline. We document the physician's orders and recommendations and ensure those tasks are implemented in the home setting.
π©βπ« We can take the time to fully educate clients and their families on their disease, the expected trajectory, their treatment options, pros and cons of different choices, and alternative therapies. Providers don't always have time during your appointment to really delve into these topics, but we can. Having more knowledge helps our clients feel in control and confident with their choices.
π’ Many local practices are doing a great job at offering specialist options under one umbrella, but all it takes is one provider outside of that EMR umbrella and you're suddenly in the fragmented health care system. How will your specialist know what your primary care provider is prescribing, and vise versa? We keep all active physicians informed of status and medication changes.
π¨βπ©βπ§βπ¦ As with all our services, we keep family informed. Family members that aren't local often worry just because they don't have information. By providing clear, through, and accurate updates from physician appointments, we can provide peace of mind to family members.
