We are a Comprehensive Home Care agency with a Home and Community-Based Services (HCBS/245D) designation, licensed by the State of Minnesota. At AG Home Health Care, we are committed to delivering high-quality, compassionate care that helps individuals remain safe, independent, and supported in their homes.

We partner with counties, clinics, hospitals, case managers, and community providers to support vulnerable adults and individuals with disabilities through reliable, compassionate, and professional care. In addition, we provide free assessment for all clients regardless of payer.
Compliance is a critical factor in delivering our services. At AG Home Health Care, we use EVV software in compliance with the Cures Act. We ensure that all of our caregivers are up-to-date and compliant with EVV laws in the state of Minnesota.
Our services include:
Basic Home Care (Non-Medical): Personal care (dressing, bathing, grooming, mobility), medication reminders, meal prep, laundry, housekeeping, companionship, and delegated health-related tasks.
245D Services: Adult companion, homemaker (cleaning, meal prep, shopping, home management), night supervision, individualized home supports, and ICLS for older adults on AC/EW programs.
Comprehensive Care: Advanced support for complex needs such as lifting/mobility assistance and medication management.
Our trained, background-checked, and TB-tested caregivers serve both waiver and private-pay clients across Dakota, Ramsey, Hennepin, Scott, Washington, Carver, and Anoka counties.

*Monitor and report changes in client condition or environment
*Engage respectfully with clients and maintain confidentiality
*Submit accurate timecards and follow company policies

Requirements:
*High school diploma (or equivalent)
*Competency test completion
*CPR/First Aid preferred
*Duties might include companionship, homemaking and personal care
*Healthcare or customer service experience a plus
*Ability to follow instructions and work independently
*Strong commitment to client care, safety, and communication
*Ability to lift, carry, push or pull max 50 lbs.
*Must be able to pass a DHS background check
