LICENSED PRACTICAL NURSE PM Shift

Conveniently located just south of the Twin Cities, The Moments Memory Care Community offers innovative design, in a vibrant atmosphere, offering clinical excellence. With this Licensed Nurse Practitioner position, you will be part of a comprehensive health services team that collectively provides a 24-Hour nurse care model for individuals with a dementia diagnosis.

LPN Position Specifications:
• The LPN is responsible for implementing the guiding direction, health services coordination, and monitoring of home care service delivery on under the Director of Nursing.
• Must have a positive attitude and innate passion for serving people living with dementia.
• Must possess the skills to communicate effectively with those you supervise, residents, family members and Director of Nursing.
• Must offer experience with leadership and team oriented supervisory skills.
• Must be able to prioritize and organize work effectively and efficiently. Attention to detail.
• Must have experience with Comprehensive Home Care and regulatory compliance.
• Ability to guide the development of a continual improvement, process-driven, best practices foundation, while building a team of exceptional care providers.
• Commitment to a compassionate care philosophy emphasizing a whole person approach.
• Excellent judgment and decision-making abilities and problem-solving skills.
• Experience with EHR/EMR (electronic health/medical records), ideally PointClickCare.
• Ability to manage all aspects of a fully integrated Memory Care program including Resident intake, Resident care, personnel management, customer relations, complaint resolution and policy development.
• Support the orientation and education requirements of CNA staff.

Other Requirements:
• Must have current LPN License registered with the Minnesota Board of Nursing.
• Must comply with a criminal background check from Department of Human Services.
• Must be screened for TB.

Certified Nursing Assistant Position Specifications:
• Responsible for providing personal care and designated health-related services designed to maintain the resident’s physical and emotional well-being.
• Assists residents with tasks of daily living.
• Must have a positive attitude and possess compassion for working with older adults.
• Must possess a passion for serving people living with dementia.

Our Dietary Aide position is designed to support a break-through culinary program and hospitality service for a leading-edge Memory Care community in Lakeville. Position Specifications:
• Uphold the standards & vision of the Culinary program at The Moments of Lakeville.
• Display empathy, positive attitude & friendliness with residents, families & visitors.
• Arrive for all shift on time, in proper uniform & eager to enrich the lives of our residents.
• Be responsible for the care of kitchen equipment, dining rooms & overall building.
• Ensure quality service, beautiful presentation & thoughtful delivery of all meals served.
• Maintain a sanitary work environment, conforming to all local codes & policies regarding proper storage, handling & serving of food.
• Maintain organization & cleanliness of all food, kitchen supply, & service ware storage areas within the community.
• Ensure all daily tasks assigned are completed to the satisfaction of the Culinary Director. This will include preparation of supplies for service, staging & cleanliness of dining areas, food service & clean-up, mechanical & manual ware washing as well as assigned shift closing duties.
• Engage residents regularly to determine dietary preferences to make the food service program individually tailored as much as possible. Refer to any dietary changes and resident preferences to the Culinary Director.
• In the event of the impending absence of shift, arrange for coverage of that shift by contacting other employees and informing the Culinary Director of change.
• Demonstrate a welcoming presence for community tours through positive/friendly interaction with all potential residents, family members and referral sources.
• Prepare dining areas and the service of food and beverage for activities, marketing and special events, as requested.
• Communicate any observed or suspected resident change of condition to the Health Services Director immediately.
• Maintain a safe & secure environment for all staff, residents & guests, following established safety standards.
• Encourage teamwork & collaboration through cooperative interactions with co-workers.
• Support a positive & professional image via actions, dress & overall dining room presentation.
