All people deserve compassionate healthcare and caring providers who will support their comfort and dignity. At A V Supportive Care and Hospice, we provide a team of compassionate and skilled professionals to help you achieve comfort and maintain dignity throughout the course of your illness. The team includes a medical doctor who personally visits and assesses you to ensure the very best care plan is designed specific to you and your needs. Nurses visit frequently to manage symptoms, medication, and ensure no new distressing symptoms occur unnoticed. Certified home health aides help patients with personal grooming and light range of motion exercises to keep the body clean and functional as long as possible. Social workers help patients and families to obtain resources needed for care, provide emotional support, and help families achieve a peaceful resolution to life and relationships. Finally, spiritual counselors come alongside patients and families to help them invoke their spiritual beliefs and practices into the care model. In some instances other specialists might be briefly included in your care plan. We provide high touch grief support to those whose loved one succumbs to their illness. We are also committed to providing veteran centric care to those who have served in the military and helping all of our patients leave a legacy to those they leave behind, whether through stories, poetry, letters or song. We care for the whole person--the physical, emotional and spiritual being. We care for the patient. We care for their loved ones and caregivers. We would be honored to care for you! Medicare.
