With all the new changes, we want to reiterate all medical records requests can still be faxed to us at 608-709-8003. Have your new primary provider office fax the request to us as we still have a few staff members who complete these.

Joe Stader and Sabeena Cheema have started at Southwest Health Center and any calls for them should be directed to 608-348-4330.

If there is something you need directly from Family Medical Center or billing inquiries you can still call our office and leave a message, a response may take up to 72 hrs.

Medical Associates Lancaster Clinic is now open! Our new Family Care Network (FCN) clinic in Lancaster, WI, provides comprehensive primary care services and convenient walk-in care. From routine checkups and preventive care to treatment for illnesses and injuries, we’re here to support your healthcare needs.

Walk-In Care Hours:
– Monday, Tuesday, Thursday, and Friday, 8 AM–5 PM (closed Wednesdays)

As of November 1, 2024, Lancaster Family Medical Center is officially closed. Congratulations to Dr. Robert Stader on his retirement and 46 years of service to healthcare!

We are also excited to officially welcome Medical Associates to Lancaster! Medical Associates, in partnership with MercyOne Dubuque, will open a new Family Care Network (FCN) clinic in our building to provide high-quality primary care services to the Lancaster community beginning Monday, December 2, 2024.

Today was the last day of regular clinic hours and we said goodbye to two of our nurses as they move onto their next adventure. We want to send many thanks to Ann and Alysha for their dedication over the last 4 years! 👩🏽‍⚕️

❗️Next week we will move into reduced hours primarily of 10 am to 3 pm and with the limited nursing staff please be patient with phone calls and other responses during this transition period.

As a reminder the last official day in clinic will be Friday, November 1st, 2024. A continuing congratulations to Bob as he nears his retirement. 🥳

Robert Stader, MD: Notice of Retirement

My medical practice began in 1969, when I asked Him to help me be accepted into medical school. The rest is history. I write this article to notify the community of my retirement and closure of Lancaster Family Medical Center on November 1, 2024.

I am tremendously grateful for all of the support and circumstances that have allowed me to care for patients over the last 46 years. Everything is a gift.

Sabeena Cheema, PA-C and Joe Stader, PA-C look forward to continuing their respective medical practices at Southwest Health Center.

Full clinic services will be available through October 4, and reduced services will be available through November 1, 2024 in an effort to smoothly transition patients. A letter to all active patients with further details on clinic closure and transferring of care is forthcoming.

Please express patience and understanding as we navigate this transition period.

Yesterday was a bittersweet day, we celebrated Renee's final day at the clinic. Renee has been a staple to Family Medical Center over the last 21 years, as a nurse and working her way to get her Nurse Practioner degree. She has been able to care and help numerous patients. Though we might be sad to have her leave, we are excited to see her continue to excel on her new adventure back in her hometown area. Thank you to all who have already sent Renee touching words of your gratitude, they are well deserved.
As we make this transition please know we are here to help with all your medical needs and still have our other 3 wonderful providers to help care for you. Feel free to reach out with any questions.
