We know that change especially in healthcare can feel stressful or uncertain. We want you to know that your care remains our top priority.
While Grand Ave Urgent Care next door is combining with the hospital system, our clinic, Grand Ave Family Medicine, is not a part of this change and will continue to operate independently. We are still here for you.
As we work through some temporary provider and scheduling adjustments, we truly appreciate your patience and understanding. Our goal through this process is to continue to serve our patients to the best of our ability and remaining rooted in our community. Please know that we are committed to clear communication and will continue to share updates, answers, and changes as soon as we are aware of them.
If you are in need of primary care services immediately through this transition phase, Nicole Alexander is happy to see you for your primary care needs, and Christina Cook will continue to see patients as usual for all mental health services.

Please note that our clinic phone number will be changing effective January 5, 2026. Our new phone number will be 307-460-5700. Our phone system will be down for a period of time on Monday, January 5th, 2026, so please be patient while we try to handle the transition as smoothly as possible.
