Taylor’s journey is a powerful reminder of how meaningful support and community care can make a life-changing difference during difficult seasons.
In October 2025, Taylor enrolled in Care Coordination during a time of significant financial uncertainty for her growing family. While expecting another baby and caring for her toddler, her husband’s unpaid job training temporarily eliminated the family’s primary source of income, placing them at risk of housing instability.
Through consistent support, advocacy, and collaboration, our team worked closely with Taylor to ensure she received timely prenatal care, connected her with critical community resources, and provided hands-on guidance every step of the way. After identifying an urgent housing concern, our team was able to help prevent eviction and provide stability for Taylor and her family during pregnancy.
This success story reflects the impact of compassionate care coordination and the importance of meeting families where they are.
