This patient with keratoconus has a history of scleral lens wear in the right eye. In addition to keratoconus, he also has glaucoma in the right eye, which further impacts his vision. Due to patient's visual demands for his job, he required sharper vision than his current scleral lens was providing. We scanned him with an OVITZ Aberrometer to assess higher order aberrations (HOAs). Higher order aberrations are complex irregularities in the eyes that are difficult to correct with regular glasses and contact lenses. They contribute to visual distortions such as ghosting, glare, double vision, etc. Based on his results, he was a good candidate for HOA corrected lenses.
HOA corrected lenses are more advanced than regular scleral lenses as they aim to correct complex visual distortions. The HOAs present in this patient's right eye with his current scleral lens are shown in the "before" picture above. Using his OVITZ aberrometry scan, HOA correction was added to the patient's lens and reduction can be seen in his aberrations indicated by the smaller bars shown in the "after" picture. This can also be seen in the last picture which shows a comparison of the aberrations present before and after addition of HOA correction.
Not only did the patient's visual symptoms improve in the HOA corrected lens, his visual acuity also improved as well. We will continue to monitor his changes yearly. It is very important that he continues his Glaucoma management as HOA does NOT fix vision loss due to optic nerve damage.
