বাংলাদেশের চিকিৎসা জগতে আমি বলতে গেলে সেই ডাইনোসর যুগের মানুষ।সেই ১৯৭৯ সালে পা রেখেছিলাম ঢাকা মেডিকেল কলেজে,আজও চিকিৎসাসেবা,চিকিৎসাবিজ্ঞান নিয়ে শিখছি,শিখাচ্ছি।
এখন আমি কাজ করছি আমেরিকার নেভাডা বিশ্ববিদ্যালয়ে কার্ডিওলজি ফেলোশিপ প্রোগ্রামের পরিচালক হিসেবে।আমি NICVD তে সর্বপ্রথম কার্ডিওলজি এমডি কোর্সে ভর্তি পরীক্ষায় প্রথম হই কিন্তু প্রফেসর আব্দুল মালিক স্যারের সুপারিশ থাকা সত্ত্বেও সেখানে নিয়োগ পাইনি।ঢাকা মেডিকেল কলেজ থেকে মেডিসিনে গোল্ড মেডেল পেয়েছিলাম,তাই একপ্রকার অভিমানেই দেশ ছেড়েছিলাম।
জীবন অনেক অদ্ভুত,কখনো কষ্টের,কখনো মজার।তবে দিনশেষে সব ভালোর জন্যই হয় বোধহয়।ভেবেছিলাম দেশেই থাকবো।কার্ডিওলজিতে যখন সুযোগ পেলাম না,তখন মন বদলালো।কমনওয়েলথ স্কলারশিপের জন্য দৌড়ানো শুরু করলাম শ্রদ্ধেয় প্রফেসর ওয়ালীউল্লাহ স্যারের সহযোগিতায়।বিধিবাম!সেখানেও বাধা পেলাম কার্ডিওলজিতে পিএইচডি করতে গিয়ে।শেষমেশ ভাবলাম জীবনে নতুন এক পথ বেছে নেই আপাতত,পরে নাহয় নিজের পথেই ফিরে আসবো আবার।সাউদাম্পটন বিশ্ববিদ্যালয়ে শুরু করলাম ক্লিনিকাল ফার্মাকোলজি নিয়ে পিএইচডি গবেষণা।এই গবেষণা কিন্তু আদতে আমার কার্ডিওলজিতে ক্যারিয়ার গড়তেও বড় ভূমিকা রেখেছিলো।ক্যালসিয়াম চ্যানেল ব্লকার নিয়ে এ গবেষণা শেষ করেছিলাম ১৯৯২ তে।অতঃপর MRCP তে উত্তীর্ণ হয়ে যাই ১৯৯৬ এ।
এরপর পেনিসিলভানিয়া বিশ্ববিদ্যালয়ে ইন্টার্নাল মেডিসিনের উপর স্নাতকোত্তর প্রশিক্ষণ নিলাম।সেখানে আমার জীবনে এক প্রবাদ পুরুষ প্রফেসর জ্যাক এন্ডির সান্নিধ্য পেলাম।তিনিই একজন মেন্টর হিসেবে আমাকে পথপ্রদর্শন করেছেন,এমনকি পরবর্তীতে ড্র্যাক্সেল বিশ্ববিদ্যালয়ে কার্ডিওভাস্কুলার ডিজিসের উপরে স্নাতকোত্তর প্রশিক্ষণ নিতেও তিনিই সুপারিশ করেছিলেন আমাকে।বলা যায় প্রফেসর জ্যাক এন্ডির হাত ধরেই আমি আমার আরাধ্য কার্ডিওলজিতে আবার পথ খুঁজে পাই।এইতো সেদিন,মার্চের ৩ তারিখ হোটেল র্যাডিসন ব্লুতে সোসাইটি অব মেডিসিনের একটি সেশনে সভাপতিত্ব করতে গিয়ে দেখি প্রফেসর জ্যাক এন্ডি সেখানে এসেছেন অতিথি বক্তা হিসেবে!বেশ আবেগাপ্লুত,কৃতজ্ঞ ও গর্বিত লাগছিলো আমার।
বিদেশ বিভূঁইতে পাড়ি জমালেও এ দেশ আমাকে সবসময়ই খুব টানে।দেশের জন্য কিছু করার ইচ্ছা থেকেই ২০০৪ সালে জাতীয় অধ্যাপক এ.কে. আজাদ স্যার যখন আমার সাথে যোগাযোগ করলেন ইব্রাহিম কার্ডিয়াকে এসে কাজ করতে,তখন বলতে গেলে আমি সুযোগটা লুফে নেই।ঢাকা মেডিকেল কলেজের একসময়ের তুখোড় ছাত্র মাকসুমুল হক ভাই আমাকে খুব আগ্রহভরে তাঁর দলে কাজ করার সুযোগ দেন।পরে রশিদ ভাই,সাইদুর রহমান খান আমাকে ওখানে কাজ করতে সহায়তা করেন।বাংলাদেশে প্রথম Rotablator,OCT(Optical Coherence Tomography)/ IVUS(Intra Vascular Ultrasound) আমরা ওখানে চালু করি।
ছাত্রজীবনে প্রফেসর ইব্রাহীম স্যার আমাদের ত্রাণকর্তা হয়ে এসেছিলেন একবার।আমি তখন কেবল ৩য় বর্ষের ছাত্র ঢাকা মেডিকেল কলেজে।আমাদের ক্যাম্পাসের জনপ্রিয় বড় ভাই,সন্ধানী চক্ষুদান সোসাইটির প্রতিষ্ঠাতা আবুল কালাম আজাদ ভাইয়ের সাথে মিলে মানুষকে রক্ত ও মরণোত্তর চক্ষু দানে উদ্বুদ্ধ করতে এক অনুষ্ঠানের আয়োজন করেছিলাম,যাতে ঠিক হয়েছিলো যে প্রধান অতিথি হিসেবে থাকবেন তৎকালীন রাষ্ট্রপতি হুসেইন মোহাম্মদ এরশাদ।সাধারণ ছাত্ররা তো কিছুতেই মেনে নিবেনা তাঁকে।এরশাদবিরোধী আন্দোলন তখন তুঙ্গে।ধরিয়ে দিলো আগুন আমাদের মঞ্চে।আমরা তো তখন নিরুপায়,অসহায়।মহৎ উদ্দেশ্য নিয়ে কাজ করতে গিয়ে এ কি বিপদে পড়লাম!আমি নিজে মরণোত্তর চক্ষুদানের পরিসংখ্যান,নীতিমালা লিখেছিলাম!শ্রীলঙ্কা হতে মরণোত্তর চক্ষুদান ও কর্নিয়া গ্রাফটিং এর অগ্রদূত ডা.হাডসন সিলভা ছিলেন আমাদের সম্মানিত অতিথি।
উপায়ন্তর না দেখে ছুটে গেলাম তৎকালীন জাতীয় অধ্যাপক ইব্রাহীম স্যারের কাছে।প্রফেসর নুরুল ইসলাম স্যার বলে দেয়ার পর প্রফেসর ইব্রাহীম স্যার তাঁর সেগুনবাগিচার বাসায় নিয়ে গিয়েছিলেন ভোরবেলায়।তারপর বর্তমানের বারডেমে নিয়ে গেলেন আমাদের।প্রথমেই তিনি আমাদের নিয়ে গেলেন হাসপাতালের ওয়াশরুম পরিদর্শনে।স্যার বলেছিলেন যে একটি হাসপাতাল কতটা ভালো,কতটা স্বাস্থ্যসম্মত ও সেবা উপযোগী তা বুঝা যায় হাসপাতালের ওয়াশরুম দেখলে।এরপরে স্যার আমাদেরকে তাঁর রুমে নিয়ে আমাদের কথা শুনলেন।হুট করেই তিনি আমাদের মেডিকেলের পড়া ধরা শুরু করলেন।আমার মনে আছে,আমাকে ক্রেবস সাইকেল নিয়ে ধরেছিলেন তিনি।তখন মেডিকেল জগতে কেবল হাঁটি হাঁটি পা পা।গলদঘর্ম হয়ে গিয়েছিলাম একেবারে!তবে হাঁফ ছেড়ে বেঁচেছিলাম যখন স্যার আমাদের একটু বাজিয়ে দেখে রাজি হয়েছিলেন সন্ধানীর অনুষ্ঠানে প্রধান অতিথি হতে।
অতীতের কথা স্মরণ করলে এমন অনেক কিংবদন্তির কথাই মনে পড়ে,আজও শ্রদ্ধায় মাথানত হয় তাঁদের জন্য।আজ আমার জায়গা থেকে আমিও প্রতিনিয়ত চেষ্টা চালিয়ে যাই দেশের জন্য কিছু করার।আমেরিকাতেই আমার নিবাস,তাও বছরে অন্তত দু-তিনবার দেশে আসাই হয় বিভিন্ন কাজে,সেমিনারে।দেশের ক্রিটিকাল চিকিৎসাসেবা,বিশেষত কার্ডিওভাস্কুলার ডিজিজ ম্যানেজমেন্টে বিকেন্দ্রীকরণ ও হেলথকেয়ারে সমন্বয় আনতে SureCare নামক একটি অ্যাপ তৈরিতে বর্তমানে কাজ করছি,যেটি অ্যাপল অ্যাপ স্টোর ও গুগল প্লেস্টোরে পাওয়া যাবে।প্রতিনিয়ত তরুণ কার্ডিওলজিস্টদের উদ্বুদ্ধ করি যেন কার্ডিওলজিকাল ম্যানেজমেন্টে Standard of care যেন দেশের মানুষ পায়।তার পূর্বশর্ত হলো Access to healthcare,Accountibility & AffordibilityAffordibility,The three "A"।প্রত্যেক চিকিৎসক যার যার জায়গা থেকে দায়বদ্ধ।তিনি দায়বদ্ধ রোগীর প্রতি,সহকর্মী চিকিৎসক ও নার্স-স্বাস্থ্যকর্মীদের প্রতি,সর্বোপরি দেশের প্রতি।আমরা কনফারেন্সে এসে আরো অনেক ব্যয়বহুল চিকিৎসার কথা শুনি,বিশেষ করে কার্ডিওলজিতে অথবা অঙ্কোলজিতে।কিন্তু সহজ প্রযুক্তি ও কিন্তু এখনো প্রত্যন্ত অঞ্চলে যায়নি।যেমন ধরুন,বাংলাদেশে সহজে বহনযোগ্য ইকোকার্ডিওগ্রাফি বা Point Of Care Ultra Sonography (POCUS) নিশ্চিত করা দরকার,যাতে করে প্রান্তিক পর্যায়ে কাঙ্খিত স্বাস্থ্যসেবা দেয়া যায়।এটি নিয়ে কাজ করে যাচ্ছি।সব মিলিয়ে চাই যেন বাংলাদেশ কার্ডিওলজিতে,চিকিৎসাসেবায় বিশ্বমানের হয়ে উঠুক।
ডাইনোসর যুগের প্রতিনিধি হিসেবে তরুণ শিক্ষার্থী,চিকিৎসকদের বলবো যত যাই করো,পড়াশোনা সবার আগে,রোগীর সেবা সবার আগে।আর রোগীর সেবা করতে গেলে তোমাদেরকে জানতে হবে,আরো জানার ইচ্ছা থাকতে হবে।বর্তমানে এই প্রতিযোগিতার যুগে গবেষণামুখী ও ব্যবহারিক প্রশিক্ষণের বিকল্প নেই।তাই জীবনের এই গুরুত্বপূর্ণ সময়টা কাজে লাগাও যথাযথভাবে যাতে বুড়োকালে তোমরাও গর্বভরে অতীতের গল্প করতে পারো,তরুণদের স্বপ্ন দেখাতে পারো,দেশের ভবিষ্যত গড়তে পারো।

In the realm of medical care in Bangladesh, I'm akin to a human of the dinosaur era. I took my first steps into the Dhaka Medical College in 1979, and to this day, I am still learning and serving in the field of medicine.
Today, I find myself as the Director of a cardiology fellowship program at the University of Nevada. Although I initially failed to get an appointment for a training post in Cardiology even after securing the first position in the admission test of first time MD Cardiology at NICVD. I accompanied Professor M Amanullah to Prof Abdul Malik's office and he kindly agreed to recommend me to the Ministry. Having received a gold medal in medicine for obtaining the highest marks in Medicine, Dhaka University, I left the country with a sense of anguish.
Life is full of twists and turns, sometimes sorrowful, sometimes joyous. But in the end, everything comes around as you stay focused with your dream and determination . I had thought I would stay in my homeland, but when cardiology didn't pan out, my heart changed. With the support of Professor Waliullah, I began pursuing a career overseas but in Cardiology. I applied for the Commonwealth scholarship. Even there, I faced obstacles in pursuing a PhD in cardiology. I thought I had to carve a new path in life, only to realize I might return to my own path later. I began a PhD research program in clinical pharmacology at Southampton University. Despite this diversion, it still held a significant role in my career in cardiology. I completed my research on calcium channel blockers in 1992, followed by the clinical training in Medicine in Liverpool and London and receiving the Membership of Royal College of London in 1994.
I moved further west. I did undertake further postgraduate training in internal medicine at the University of Pennsylvania in Philadelphia. . There, I encountered my mentor Professor Jack Ende, who guided me along the way. He recommended me for further training in cardiovascular diseases at Drexel University. With his guidance, I found my way back to the field, I loved most, cardiology. It was on this March 3rd, 2024 at a session of the Society of Medicine held at the Radisson Blu hotel, Dhaka that I myself was supposed to preside, I found Professor Jack Ende as a guest speaker! I was filled with excitement, gratitude, and pride.
Even as I ventured abroad, my heart remained tied to my motherland. In 2004, when National Professor, Professor A.K. Azad contacted me to work at Ibrahim Cardiac Center. I was over the moon not to miss that opportunity. One of the brilliant students of DMC, and my senior, Prof Maksumul Hoque bhai,later Prof Rashid bhai and Prof Sayeedur Rahman Khan Mishu, h had helped me work there. We introduced procedures like Rotablator,OCT (Optical Coherence Tomograph), IVUS (Intravascular Ultrasound) for the first time in Bangladesh.
Professor Ibrahim became our saviour once in our student days. I was only a third-year student at Dhaka Medical College then. Together with famous Abul Kalam Azad bhai, the founder of Shandhani Eye Society, we organized an event to raise awareness about blood donation and post-humous eye donation. The then-President Hussein Muhammad Ershad was invited to be the chief guest to get media attention and give the message to the people to promote the cause. But this triggered an uproar and fierce rng the students. We were facing a crisis that stunned usus and made us sad. I remember that I did draft the paper on statistics and practicalities and guidelines for posthumous eye donation and now the program at stake, it was hurting us. Dr Hudson Silva from Sri Lanka, a pioneer in promoting post humous eye donation and for successful cornea grafting was the guest of honor.
When things got tough, we sought help from Professor Ibrahim. We went to another legend of our time, Professor Nurul Islam who had suggested that we go to meet Professor Ibrahim. Prof Ibrahim asked us to see him in his Segunbagicha home early in the morning. Later, he brought us to the present BIRDEM. He started by taking us to inspect the hospital's washrooms. He said the condition of a hospital's washrooms reflects its quality of care. Then, he listened to our stories in his room. I remember him holding me accountable for my studies, he even asked me about Krebs Cycle! Cellular turn over of the endothelial cells of small intestine. It was a sigh of relief that he agreed to be our Chief guest. The event was saved
Remembering the past, many illustrious stories come to mind. I still hold them in reverence today. From my position, I strive to contribute to the country every day. Although I reside in America, I return to Bangladesh several times a year for various seminars and projects. I am currently working on creating an app called "SureCare" available in Apple App store and Google playstore, to decentralize & integrate critical cardiac care and healthcare services. I constantly encourage young cardiologists to ensure the standard of care in critical cardiovascular management. For this the health care system must be accessible, accountable, and affordable in Bangladesh, the three “A’’s.
Every physician is accountable to his or her conscience, friends and colleagues, peers and to patients and their families. For this, we need to see the empathy in our youngsters. We talk about embracing new technology, more expensive technology and ensuring standard of care and advancements in healthcare but I feel it is equally important to make sure the technology and advancements reach the people in remote areas or to cities beyond the capital. One such example is to have point of Care Ultrasound which is easily portable. Remote areas need access to these Point-Of-Care technologies and services and these can be the gateways to connect to higher care centers for the specialist services in different disciplines. . I am working towards this goal. Together, let's ensure that our people can get the care needed.
As a representative of the dinosaur era, I urge young students and doctors to prioritize learning and patient care above everything else. To serve the patients, you must understand them and have the desire to learn more. In this age of competition, there is no other alternative or short cut. One needs to be prepared fully in clinical medicine, research-oriented and undertake all the practical hands on training. We can not afford to waste time, our youngsters need to utilize the time of life when in the medical school and in training effectively so that you can materialize the dream for the future, and shape the destiny of our country and proudly recount stories of the past,
