In addition, we provide special packaging (ex: bubble packs or bingo cards) for these patients to remain compliant with state regulations and improve patient medication adherence. We improve patient compliance with monthly refill reminder programs, as well as providing the facilities with MARs (medication administration record). We work closely with family members and provide billing statements monthly to their loved ones. HOSPITAL SERVICES - MEDICATION THERAPY MANAGEMENT (MTM)

What are medication therapy management services? Medication therapy management (MTM) is a distinct service or group of services that optimize drug therapy with the intent of improved therapeutic outcomes for individual patients. Services include a broad range of professional activities, including but not limited to performing patient assessment and/or a comprehensive medication review, formulating a medication treatment plan, monitoring efficacy and safety of medication therapy, enhancing medication adherence through patient empowerment and education, and documenting and communicating MTM services to prescribers in order to maintain comprehensive patient care. Medication therapy management includes five core components: a medication therapy review (MTR), personal medication record (PMR), medication-related action plan (MAP), intervention and/or referral, and documentation and follow-up. A systematic process of collecting patient and medication-related information which occurs during the pharmacist-patient encounter. In addition, the review assists in the identification and prioritization of medication-related problems. During the MTM encounter, the pharmacist develops a PMR for use by the patient. What is the benefit of receiving a comprehensive medication review? Medication-related problems and medication mismanagement are a massive public health problem in the United States. Expenses associated with treating medication-related adverse events resulting in hospitalizations doctor visits, lab exams, etc., cost the economy over $200 billion every year. Patients are not immune and responsible for their copays, along with their general well being. Patients on average take 11.4 meds/day. It is a daunting task to manage, correctly administer, understand the role of medications, and understand the disease state. An estimated 12% of the U.S. population is considered to be health literate. NOW SERVICING ALL YOUR NON STERILE COMPOUND NEEDS

Compounding Pharmacy

Do compounded medications require FDA Approval? The FDA approval process is intended for mass-produced drugs made by large manufacturers. Since compounded medications are personalized for individual patients’ needs, it is not possible for each formulation to go through the FDA’s drug approval process. This approval process takes years to complete and is prohibitively expensive, often costing millions of dollars. At Well Care Compounding Pharmacy, we understand that there is no such thing as a one-size-fits-all solution for a patient’s pharmaceutical needs. A patient may be allergic or sensitive to dyes or fillers in traditional medications- or that patient may just have trouble swallowing pills. We use the most advanced compounding techniques to prepare medications that suit any patient’s specific needs. We are able to prepare compounds in a wide variety of dosage forms including liquids, special flavorings, troches, creams, ointments, sprays, suppositories, and much, much more. Our specially-trained Pharmacists and technicians are able to compound prescriptions designed uniquely for you. Our different services include, but are not limited to:

Synchronizing medication can greatly benefit the elderly population in several ways.

Firstly, it helps to improve medication adherence. Many elderly individuals take multiple medications throughout the day, and keeping track of different schedules can be challenging. By synchronizing their medications, they can align all their doses to be taken at the same time, making it easier to remember and follow their medication regimen. This reduces the risk of missed or incorrect doses, improving overall medication adherence and effectiveness.

Additionally, synchronization simplifies medication management for both the elderly individuals and their caregivers. Instead of dealing with multiple refill dates and different medication schedules, synchronizing medications allows for a single pickup and coordinated schedule. This streamlines the process, reduces confusion, and minimizes the chance of errors or mix-ups.

Synchronization also helps to avoid potential drug interactions. When medications are taken at different times, there is a greater risk of unintended interactions between medications. By synchronizing medication schedules, healthcare professionals can better monitor and manage the medications to ensure they are compatible and do not pose any risks to the elderly patient’s health.

Flu Facts vs. Myths

❌You can't spread the flu if you're feeling well.
❎Actually, 20% to 30% of people carrying the influenza virus have no symptoms.

❌Healthy people don't need to be vaccinated.
❎ While it's especially important for people who have a chronic illness to get the flu shot, anyone — even healthy folks — can benefit from being vaccinated.

❌You don't need to get a flu shot every year.
❎ The influenza virus changes (mutates) each year. So, getting vaccinated each year is important to make sure you have immunity to the strains most likely to cause an outbreak.

❌ The flu vaccine gives you the flu.
❎The flu shot is made from an inactivated virus that can't transmit infection. So, people who get sick after receiving a flu vaccination were going to get sick anyway.

A pharmacist will sit down with you and review every prescription and over the counter medication, including supplements and alternative therapies.
