Horizon Specialty Hospital is a long-term acute care hospital for patients requiring extended hospitalization to meet the medical and rehabilitation needs. Independent physicians visit daily to evaluate and manage patient care and the intense services of a multi disciplinary team of professionals.

Individual goals for each patient are assessed and planned for from the day of admission to patient discharge home or to a lower level of care.
