🌿Let’s talk about the limits of prognostic certainty in pregnancy and childbirth.

❌Some diagnoses and conditions are absolute, but that category is more rare than obstetrics in routine hospital-based care lead us to believe.

💯Legal and ethical principles of informed consent should always guide us. The most common and often overlooked use of informed consent happens in the largest area where clinical recommendations are made based on the ✨best available evidence,✨ though without prognostic certainty.

🌿Informed consent about pregnancy and birth issues that involve known risks are not quickies. These conversations and decisions often evolve over more than one visit. Careful honesty, centering the patient, and knowledge about alternatives are some of the most important features of these conversations.

💜Midwives know that we all have different values and belief systems. Everybody deserves ethical and legal informed consent in their care.

Midwives and birth workers have big hearts! Many of us also love dogs and care deeply about animal welfare. 💗

We’re happy to share our first Serenity volunteer event at the Joseph Mayo Rehabilitation ReTreat . It was a morning full of doggy car rides, walks, park visits, lots of petting, hugs, and love.

🐾 All of the rehabilitation, training, and enrichment activities help these dogs shine to be their most adoptable selves so every one of them can have their forever homes.

Our next volunteer event will be at the HAV Cat Cafe, and we will definitely be back to love on the ReTreat dogs again!

Why can’t clinicians agree about water birth safety?

“According to The Diffusion of Innovation Model, an innovation that does not adhere to the existing values will not be diffused. These results suggest that the evidence authored by midwives and nurses does not fit within the values of physicians” (Vanderlaan, Kamanga, & Garcia, 2022).

Please consider these study results to see why water birth is so poorly understood across nurses, midwives, obstetricians, and pediatricians. This conflict continues among perinatal disciplines and individual clinicians, despite increasing evidence about safety and maternal satisfaction with water birth experiences.

Midwives and nurses published 60% of their articles about water birth in midwifery or nursing journals. Meanwhile pediatrician authors referenced articles written by midwives or nurses for only 20% of their referenced citations, despite midwives and nurses contributing over one-third of the primary research articles in this sample.
In addition, authors from pediatric specialties referenced commentaries and case studies in their publications more often than they cited primary research (77.7% vs 14.8%).

I was invited to join this systematic review of scholarly references about water immersion for labor and/or birth by one of my research mentors, Jennifer Vanderlaan, . It remains too relevant and explains a lot about inconsistencies in knowledge and beliefs about water birth and other practices among perinatal specialists.

We co-authored this study as a midwife and obstetrician team to show a level of interdisciplinary and inter-facility commitment between our patients at Serenity birth center and our designated transfer hospital 

The midwifery-led model of care and community birth offer better outcomes for appropriately risked women who want this model of care and birth experience.

“One of the most important factors to protect patient safety during planned community birth at home or in a birth center is the quality and efficiency of hospital transfers when emergent or emergency risks arise in a previously low-risk woman or newborn” (Garcia & Matsunaga-Kirgan, 2026).

Successful transfer processes also include regular education and drills with EMS and our fire and paramedic stations for efficient and skilled continuation of care in transit while the midwife or perinatal RN provide ongoing, specialized care until the transfer is complete.
