This week, we are celebrating our Nurse Case Managers, Social Workers and Discharge Planners! The goal of Case Management is to help individuals achieve their highest possible level of wellness and autonomy by connecting them with resources and navigating complex service systems.

At Spring Valley Hospital, some of our staff have advanced certifications in Case Management, some of our social workers have specialized clinical license designations and all of our social workers are master's level-prepared. Thank you for all you do!
