Mission Statement
Our Mission is to effect changes in a person’s ability to deal with his/her environmental challenges by providing quality mental health and substance abuse services using a holistic approach. It is our overall goal to enhance the quality of life for individuals and families. is a Non-for-profit 501(C)3 Community Mental Health Center, designed to assist the mental health population of South Florida. We believe in aiding those we serve with love, compassion, and respect. Who are we:
Community Connections Psychosocial Rehabilitation (PSR) emerged from the needs of the local community. Our organization provides a vast array of services such as Targeted Case Management, Substance Abuse Services, counseling within the community, Clothing bank, Food drives, connection to housing, youth internships and participates in community event with several community partners. PSR focuses on holistically enhancing living skills for individuals with severe and persistent mental illness to transition them back into the community. The PSR service is a 6 to 12 month program, available 5 days a week and is tailored to the client’s individual needs. We provide a therapeutic, warm, inviting, exciting, and safe environment. Our belief is that all people are valuable and unique and should be treated with dignity and respect. While recognizing the potential for change, by assessing of the client’s emotional, physical, spiritual, and life experience is provided in a caring environment. The growth of the individual is promoted through a course of treatment developed and executed in a timely manner. Clients are individually linked to a Targeted Case Manager that provides one-to-one services to help rediscover their independence. Goals are customized with options to help them select their services, service providers, and service outcomes. Every day we aim to make a difference in the lives of those we serve by meeting their unique needs. Our services are always open and easily accessible. We are conveniently located to service Miami-Dade, Broward, and Palm Beach Counties. Who we serve:
Services are limited to Medicaid recipients who have a serious emotional disturbance or chronic mental illness. Such as: Generalized Anxiety Disorder (GAD), Substance Abuse Disorder (SAD),
Co-occurring Disorder (COD), Personality Disorder, Bipolar, Schizophrenia, Post Traumatic Stress Disorder (PTSD), Attention Deficit Hyperactivity Disorder (ADHD), Obsessive Compulsive Disorder (OCD), Oppositional Defiant Disorder (ODD). We provide the following services:
• Individual/ Family Therapy
• Screening-Intake Service
• Service Plan Development
• Medication Management
• Education
• Job Training
• Psycho-Social Rehabilitation
• Pharmacy Services
• Discharge Planning
• Case Management
• Family Support
• AA/NA
• Roundtrip Transportation to and from Program

• Medical & Psychiatric Services
• On-Call Nurse
• Clothing/ Food Bank Services
• Assistants with Applying for Food Stamps
• Breakfast & Lunch provided during program hours
