Case Manager & Patient Coordinator

Looking to make a real difference in people’s lives? We’re seeking a compassionate, organized Case Manager / Client Services Coordinator to join our team.

What you’ll do:
• Coordinate enrollment and community services for participants
• Document service coordination notes
• Manage authorization renewals and follow-ups
• Submit required documentation to insurance plans and track approvals
• Communicate effectively with Spanish and English speaking participants, caregivers, and providers
• Advocate for participant needs with professionalism and empathy

What we’re looking for:
• Fluent in English and Spanish (required)
• Strong communication and organizational skills
• Experience in healthcare, social services, or care coordination preferred
• Compassionate, dependable, and detail-oriented with strong follow-up skills
• Ability to multitask in a fast-paced environment
