Heartland Community Health Center Comprehensive primary care, dental, and mental health for all, regardless of income or insurance status. Heartland accepts both insured and uninsured patients.

Uninsured patients pay on a sliding fee scale, which makes quality care affordable at Heartland. Heartland participates in the Increase the Reach program. This Increase the Reach grant program is supported by the Centers for Disease Control and Prevention (CDC) of the U.S. Department of Health and Human Services (HHS) as part of a financial assistance award totaling $79,278,482 with 100 percent funded by the CDC/HHS. Daniel, Victor, Hannah, and Jim. If you've been following Heartland you may have heard these names before. These are real patients who are shining examples of the benefits of integrated care.

- Daniel struggled with mental health and substance use disorder.
- Victor was experiencing long-term homelessness, chronic health issues, and substance use disorder.
- Hannah was receiving her primary care at Panda Pediatrics when her mother died from Covid-19, causing her to feel lost and depressed.
- Jim was facing physical and emotional issues that impacted his health and his relationships.

Poor oral health can be linked to a variety of health issues, and dental care can be out of reach for people without private insurance. This donation will go a long way toward ensuring many veterans in our community can access the dental care services they need.

Heartland Community Health Center received a $15,000 donation from the Lawrence Veterans Day Parade Association to help provide care for veterans in need. 2025: The Year of Community Health.

This year, Heartland leaned even deeper into care that reaches beyond clinic walls.
• Community Health Worker services grew by 48%, supporting 890 patients and families
• More than 80% of CHW patients were also connected to medical care, strengthening coordination and follow-up

Across all services, nearly 23,000 patients received care in 2025.

Care stayed connected:
• 6,571 patients received care from more than one department
• Around 1 in 3 patients benefited from integrated, team-based care

Behavioral health services grew, expanding access to therapy and psychiatry for nearly 800 additional patients.

Through Food is Medicine classes and the Care Cupboard, patients and families also received support with healthy food access, nutrition education, and meeting basic needs that directly impact health.

Recognized through Best of Lawrence and supported by thousands who showed up through events like the Kansas Half Marathon/5k, this was a year powered by community.

The Care Cupboard provides healthy food and nutrition education for more than 2,000 local families each year. Every $1 you give is stretched with donations & partnerships to supply $25 in food to the people who need it most.
