The moment you have all been waiting for is here . . . Lawrence Family Medicine has a new and improved Patient Portal! Please call (785-331-1700) or e-mail (receptionist@lfmo.com) to sign-up for access. If e-mailing your request to sign-up, please include your full name, date of birth, and the e-mail address you would like to receive alerts through. If other members of your family are patients of ours as well, please include their names and dates of birth. Once we receive your request, we will send you an e-mail with a temporary username and password that you can use to log in to your account. Once logged in for the first time, you will be able to change your username and password.

Through the portal, you will be able to request medication refills, contact your doctor’s nurse, ask billing or scheduling questions, and much more!

**We are using a completely new platform, so you will need to re-register even if you had an account on our old portal.

Your best protection from COVID-19 will be a combination of getting a COVID-19 vaccine, wearing a mask, staying at least 6 feet away from others, avoiding crowds poorly and ventilated spaces, and washing your hands often. COVID Vaccine appointments are available (Tues-Thurs) at Douglas County Fairgrounds for anyone 16 and older. Enter the code for the date you select at LDCHealth.org/getmyvaccine

Plenty of 1st dose appointments available for all days the next two weeks.

COVID-19 vaccination for individuals eligible in phase 3 & 4 will begin next Monday (03/22/2021)!

If you are interested in getting the vaccine and have not already done so, please complete the Douglas County online survey that can be found on this website (https://ldchealth.org/460/COVID-19-Vaccine). You can also double check to see if you completed the form correctly with their vaccine interest form search found on that same website. Completing the survey does not create an
appointment or reserve a vaccine, but it will help planners identify which phase individuals qualify for and help streamline the notification and scheduling process as more vaccine doses
become available.

If residents need assistance filling out the form, please call the Senior Resource Center for Douglas County at 785-842-0543 and leave a message. Staff will return calls during business
hours as quickly as they can in the order calls are received.

Included in phases 3 & 4 are those aged 16 through 64 with conditions that increase the risk for severe illness from COVID-19, including:

· Cancer
· Down Syndrome
· Certain Heart Conditions
· Type 1 & 2 Diabetes
· Pregnancy
· Asthma
· Cystic Fibrosis
· Liver Disease
· Neurological conditions like dementia
· Other medical risks

Also included in phases 3 & 4 are other non-health workers in critical infrastructure who cannot work remotely, including:

· Agricultural and food workers not included in previous phases
· Workers performing in-person activities indoors
· Utility workers
· Social service and government workers not included in previous phases
· Logistics workers, such as truck transportation workers and couriers
· Water and wastewater workers
· Shelter, housing and finance workers
· Information technology and communication workers.

The following pharmacies are also participating in the Douglas County COVID-19 vaccine effort: Dillon's, Medical Arts, Orchards Drug, Sigler, and Wal-Mart. Please CONTACT THE PHARMACY
