"Acetaminophen has been the drug of choice for treatment of fever and during pregnancy for decades.
The US Department of Health and Human Services has issued an announcement expressing concern about a potential link between acetaminophen exposure in utero and neurodevelopmental disorders such as autism.
Data from observational studies have been conflicting, but higher-quality studies have been less likely to show an association between maternal acetaminophen use and neurodevelopmental disorders in their offspring.
Fever during pregnancy has itself been associated with adverse neurodevelopmental outcomes, and it can cause neural tube defects during the first trimester.
Alternative drugs for treatment of fever or pain (e.g., NSAIDs, opiods) have been associated with significant toxicity when used during pregnancy.
Major professional organizations continue to recommend acetaminophen for first-line treatment of fever and pain during pregnancy."

November '25 Health Tip - Interstitial Cystitis Bladder Testing and Treatment

Do you have “UTI” symptoms but no infection?

Many problems can cause the same symptoms as a bladder (UTI) infection. Dr. Saxer can evaluate these symptoms to determine the cause and best course of treatment. Among the possible problems is interstitial cystitis, which is an inflammation or hypersensitive bladder. This can be tested for in the office. It usually occurs in women, but sometimes men also have the problem.

For women, you would get into the “PAP position” and rest your legs on leg crutches. The bladder is catheterized by sterile technique and any remaining urine is drained. One of two solutions is passed into the bladder and you report whether you have any symptoms. That solution is drained and the second solution is passed into the bladder to see if you experience any symptoms. The catheter is then removed from the bladder and you can urinate the remaining fluid. Dr. Saxer will discuss the test information and a treatment plan with you.

Those who have interstitial cystitis frequently benefit from putting medicine into the bladder. The bladder is catheterized and medication is passed through the catheter into the bladder. The catheter is removed and the medication is left in the bladder to help treat it.

October '25 Health Tip - Incision and Drainage of Abscesses and Thrombosed Hemorrhoids

Do you have a very painful abscess, boil or hemorrhoid?

These can frequently be “lanced” or incised and drained to allow healing. Usually the area is cleansed and local anesthetic (numbing medicine) is injected. A scalpel or scissors is then used to open the abscess or hemorrhoid. Once the pus or clot is removed from inside the lesion, Dr. Saxer may need to remove some or all of the lining to help prevent recurrences. With abscesses, he may also pack the space with a cloth-like material to help remove further contents and allow it to heal faster. If a packing is placed, you or Dr. Saxer may need to replace it over the next several days. Antibiotics and pain meds may also need to be prescribed, and you’ll schedule a recheck.

September '25 Health Tip - Hysteroscopy

Do you have abnormal vaginal bleeding, a PAP that has concerning uterine cells, or fertility problems?

Dr. Saxer may help you by performing a hysteroscopy. This procedure allows him to view the inside of the uterus with a scope. It usually accompanies an endometrial biopsy in the office or a dilation and curettage (D&C) in the hospital. Not many physicians have the flexible scope available in their offices that Dr. Saxer has. Performing this procedure in the office can prevent the need for a D&C in the hospital under general anesthesia.

In the office, you would get into the “PAP position” and your legs can have extra support from the exam table’s leg crutches. The cervix area is prepped with a cleansing liquid and a paracervical anesthetic block is placed to assist with comfort. The small, flexible scope is passed through the cervix to look at the inside of the uterus. Polyps or tumors that may not be seen well with an ultrasound or abnormal contours to the surface are visible with the hysteroscope. An endometrial biopsy is frequently then performed to test the cells. If this is performed along with a D&C in the hospital, a thicker and rigid or straight hysteroscope is used after the dilation (D) and before the curettage (C). After the procedure is completed, you would return for a recheck in one week.

We are all more than just a part or a sum of parts. We are all wholly individuals.

I saw a 13 year old female recently who had been seeing subspecialty care for years, had several procedures and was put on complicated treatment regimens. She was not improving. We reviewed her problems, examined her, considered other simpler tests not performed yet and simplified her medication approach. I pulled experience from several other specialties to approach her problems. We have many other options that were not tried in all those years, and (with His help from above) I am confident, she will improve.

August '25 Health Tip - Fine Needle Biopsy of Breast or Thyroid

If there is a lump in your breast or thyroid, Dr. Saxer can perform a fine needle biopsy to help determine what kind of lump it is.
For this procedure, the area is prepped with a sterilizing agent. Using sterile technique, a needle attached to a syringe is placed into the lump to pull out cells. This action is usually performed a few times to get a good specimen. These cells are then sent to the lab for evaluation.

Cardiovascular Risks on Life Expectancy - Hypertension ( High Blood Pressure), Hyperlipidemia (High Cholesterol), Obesity, Diabetes (High Blood Sugars), and Smoking all increase the number one killer in America-cardiovascular death (Heart Attack or Stroke). Having all of these uncontrolled risks decreases your life expectancy by 10 years as was found in the recent article in New England Journal of Medicine.

Dr. Saxer works hard to eliminate or control each of these risks and his business model can help better than the health insurance world. A patient from his original practice had put off coming to see Dr. Saxer since he did not take insurance. This patient made an appointment with Dr. Saxer recently and was found to have at least 3 risk factors not controlled. Since he could make an appointment at JSFMC that gave him the time that he needed, we were able to work to control these risks and prolong his life expectancy.

JSFMC is a better approach to care (and most patients have health insurance).

July '25 Health Tip - Evacuation of Subungual Hematoma

Summer finds us doing more activities that risk injuring our fingers or toes.

If blood forms under the nail, it increases the pain as it pushes on the skin and nail. Usually if a hole is placed in the nail before three days have gone by, much of the pressure is relieved. Dr. Saxer usually places a digital nerve block (similar to what the dentist does for dental work) at the base of the finger or toe for comfort. The nail is then cleansed and a needle is used to bore a hole in the nail to release the blood and pressure. Most people lose their nail due to the trauma of hitting it so hard. The good news is that it usually grows back nicely.
