I began my cardiopulmonary examination by partially lowering her hospital gown. It immediately became apparent that she had a dermatomal vesicular rash of herpes zoster. As a family physician in the fourth decade of practice, I have seen many changes in medicine. Unfortunately, the insurance companies are significantly damaging healthcare. They are practicing medicine without a license but are protected by their “benefit” status. The insurance companies are increasing costs and interfering with providing care. Our country has the most expensive and among the worst in quality of industrialized countries. We need more empowered family physicians to practice cost effective medicine as in other countries than paper push for the insurance companies.
My career started as a solo practitioner, then physician/administrator of a small group, then employed in corporate employee health, and now back to solo practice but without accepting insurance. I base my care, including surgeries, on time and am able to charge much less than when I accepted insurance. Plus, my overhead used to be 1/3 of my gross income having workers in the office to interface with the insurance companies.
I was asked to be one of the founding physicians for the Kansas City beta site for Sesame Care. Sesame Care was founded by David Goldhill who wrote “Catastrophic Care” that I recommend to all who are working on improving healthcare. This company does not participate with insurance and works to keep costs down and quality high.
Direct Primary Care (DPC) is another effort for physicians to practice medicine without the interference of the insurance companies.
Please know that your efforts are appreciated and with your platform, you can help millions. There are so many facets to healthcare. We need caring people in positions to be able to see and act on those areas that can have the most impact.

Nothing in medicine has saved more lives than vaccinations!

Vaccinations has become a controversial topic due to much misinformation. At this time, your physician who is relying on scientific truths is your best source of accurate information for you. We are seeing increases in diseases thought to have been extinct since people are not getting their vaccines. These diseases cause significant morbidity and mortality.

We must get through this together for your health and those you love.

Moonset - "Strategies to Reinvigorate the Bedside Clinical Encounter"

From the NEJM 11/27/25 issue, "Decreased bedside clinical skills lead to diagnostic errors, poor clinical outcomes, increased health care costs, and physician burnout." Also, "Medical trainees today spend as little as 13% of their time in direct contact with patients...More than half of the outpatient diagnostic errors have been attributed to poor history taking and mistakes in the physical examination. An overreliance on technology, due in part to declining clinical skills, leads to overinvestigation and rising costs...It also leads to a weakening of the doctor-patient relationship. Lack of time at the bedside disproportionally affects marginalized groups and propagates health care disparities."

I'm glad to see that authorities in medicine are working on this aspect in the decline in medical care we see today. My thoroughness in taking medical histories and providing physical examination have been able to expose the true cause of underlying medical problems for proper treatment in many who have been improperly cared for. Comprehensive annual physical examinations also can detect early problems before they are no longer curable.

This is also why I have expressed my concern on people's reliance on telehealth. I recently saw a male who had been treated for prostatitis by a telehealth physician. When he was not improving, the physician recommended he should be seen in person. He had bilateral inguinal hernias as his cause for discomfort.

Aerobic exercise - aim for 30 minutes per day, 6-7 days per week
Diet - such as the Mediterranean diet, especially a diet high in flavanols/flavonoids with fruits, vegetables and nuts
Social Activity
New learning intellectual activity
Higher education
Sleep - at least 7.5 hours per night
Good control of cholesterol, blood pressure and blood sugars
Avoid smoking
Avoid excessive alcohol intake

URI - Acute Upper Respiratory Infections
Cold and Flu season is upon us. There are many causes of fever, runny nose, cough, achiness, sore throat, chills, head or chest congestion, ear pressure and/or being tired. The children are back in school. The next peak will be Thanksgiving time and Christmas time. Wash your hands or use hand sanitizer before touching your face.
Symptomatic treatment includes rest, extra fluids, vaporizer in your bedroom, Tylenol, lozenges, Mucinex, Zinc, Sudafed, or ibuprofen/naproxen if no stomach problems. Zinc is one of the few treatments that has actually shown to decrease the severity and length of a cold. There are lozenges or some take the 50 mg tablets twice each day. I have found Mucinex can decrease sinusitis and bronchitis sequalae. Sudafed is the only true decongestant that can be obtained behind the counter (for those without high blood pressure or insomnia or other reasons to avoid it).

"Acetaminophen has been the drug of choice for treatment of fever and during pregnancy for decades.
The US Department of Health and Human Services has issued an announcement expressing concern about a potential link between acetaminophen exposure in utero and neurodevelopmental disorders such as autism.
Data from observational studies have been conflicting, but higher-quality studies have been less likely to show an association between maternal acetaminophen use and neurodevelopmental disorders in their offspring.
Fever during pregnancy has itself been associated with adverse neurodevelopmental outcomes, and it can cause neural tube defects during the first trimester.
Alternative drugs for treatment of fever or pain (e.g., NSAIDs, opiods) have been associated with significant toxicity when used during pregnancy.
Major professional organizations continue to recommend acetaminophen for first-line treatment of fever and pain during pregnancy."

Are you a person that develops bad scars? You are not alone!

November '25 Health Tip - Interstitial Cystitis Bladder Testing and Treatment

Do you have “UTI” symptoms but no infection?

Many problems can cause the same symptoms as a bladder (UTI) infection. Dr. Saxer can evaluate these symptoms to determine the cause and best course of treatment. Among the possible problems is interstitial cystitis, which is an inflammation or hypersensitive bladder. This can be tested for in the office. It usually occurs in women, but sometimes men also have the problem.

For women, you would get into the “PAP position” and rest your legs on leg crutches. The bladder is catheterized by sterile technique and any remaining urine is drained. One of two solutions is passed into the bladder and you report whether you have any symptoms. That solution is drained and the second solution is passed into the bladder to see if you experience any symptoms. The catheter is then removed from the bladder and you can urinate the remaining fluid. Dr. Saxer will discuss the test information and a treatment plan with you.

Those who have interstitial cystitis frequently benefit from putting medicine into the bladder. The bladder is catheterized and medication is passed through the catheter into the bladder. The catheter is removed and the medication is left in the bladder to help treat it.

Dr. Saxer performs many procedures for his patients to help diagnose or treat medical issues. If you need any procedure, make an appointment with Dr. Saxer. His fees are less than what you would pay through your insurance.
