Please fill out the highlighted portion before you drop off the form to the front office.

If you would like the form faxed please sign the bottom portion and provide the name of the school it needs to go to.

Looking for applicants that can multitask, work well with others, reliable to come to work, and comfortable working with children of all ages.

No certification required, experience taking vitals is preferred.

Forms you may need include:

Physical form
Immunization form
Prescription Medication form
Over The Counter Medication form
Dietary restrictions and allergy accommodation forms
Sports physical forms

Not all of these apply to every child, please check with your school district for what you might need.

If your child does not attend Grayson County Schools please provide the appropriate form from your district.

Forms are available in 3-5 business days.

Sports physicals are always included with a yearly well exam and are good for 13 months. If your child has had a well exam in the last year and you need sports physical paper work please contact the office at 270-259-5641 to fill out the parent portion of the form and then the provider will fill out their portion of the form.

Next day appointment available, feel free to call the office for all your physical needs.
