Leitchfield Pediatrics Leitchfield Pediatrics is an accredited Rural Health Clinic. Caring for Infants, Children and Adole We all have children in our lives that we love and care for.

Here at Leitchfield Pediatrics we are all fathers, mothers, aunts, uncles, brothers, sisters and godparents. We want to assure you that when we are taking care of your children, we ask the same question every time: "Is this what I would want done for my own?" If that answer isn't yes, then we don't do it. It's important to us that you both understand and feel comfortable with your child's care. So feel free to ask us questions at any time! We provide high quality, comprehensive care for newborns, infants, children and adolescents. If you are looking for a pediatrician for your children, we are always accepting new patients and would love to meet you!

School is just around the corner!

Forms you may need include:

Physical form
Immunization form
Prescription Medication form
Over The Counter Medication form
Dietary restrictions and allergy accommodation forms
Sports physical forms

Not all of these apply to every child, please check with your school district for what you might need.

If your child does not attend Grayson County Schools please provide the appropriate form from your district.

Forms are available in 3-5 business days.
