A.I.M. Wellness is currently seeking a Community Support Associate (CSA) to join our growing team in Leitchfield! If you are passionate about helping others, making a difference in the community, and working in a supportive, team-oriented environment, we would love to hear from you.
✨ Position Overview:
Community Support Associates provide one-on-one and group support to patients in community and home-based settings. This role focuses on helping individuals build daily living skills, connect with community resources, and work toward their treatment goals while promoting a safe, respectful, and supportive environment.
🧠 Key Responsibilities Include:
• Supporting patients with daily living skills (budgeting, cooking, personal care, transportation)
• Assisting patients with accessing community resources such as housing, employment, healthcare, and education
• Documenting patient interactions and progress notes per agency and regulatory standards
• Collaborating with therapists and service coordinators to ensure continuity of care
• Monitoring progress toward treatment goals and adjusting supports as needed
• Participating in trainings and professional development opportunities
📌 Requirements:
• High School Diploma or GED
