RN ~ Clinical Liaison

North Country Associates is in search of a Clinical Liaison to manage and develop clinical referrals from our hospital accounts for patients needing skilled nursing services as well as Long Term Care. This position will be responsible for assessing clinical appropriateness for admissions as well as to facilitate all aspects of a seamless transition from acute to sub-acute care.

The ideal candidate will be able to work independently as well as with a team and have strong communication skills. We are looking for a candidate that has the ability to build strong relationships with our healthcare partners.

This full time (6 months) program is designedto prepare an individual for a challenging career as an Administrator in a North Country Associates’ nursing facility.

Qualified individuals must:
* Possess a minimum of a Bachelor’s Degree in a business or health care field
* Possess previous experience in a health care setting
* Be an energetic self-starter
* Have supervisory experience with demonstrated knowledge of budget systems
* Be willing to relocate as part of the training and placement program
