we generally know to go to the doctor or PT when we have pain with or difficulty moving our arm or leg. If there is knee pain/elbow pain or suddenly i cant bend my elbow - we go and get help. Why do we not think of the pelvic floor the same way?

Because the pelvic floor doesnt move a joint - i think this messes with peoples mind. The pelvic floor is skeletal muscle just like the bicep or quad so experiences the same issues at honestly a significantly higher rate. But when it needs help, it generally manifests it self in issues with peeing, pooping and s*x. (its main jobs).

When it needs help, it can be complex (more than just do kegels) it can need more motion, coordination, need to be stretched, strengthening or sometimes surgical intervention.

There are people out there that know this part of the body well. That dont think what you are experiencing is weird or gross. Nothing is TMI.

Some studies show 1 in 2 women who deliver va**nally will experience pelvic organ prolapse.

Not 1 in 10. Not a rare complication. Half. And most of them will spend years thinking the heaviness, the pressure, the bulge, the feeling that something is falling out is just what happens after you have babies.

It is not just what happens. And you do not have to live with it.

Prolapse occurs when the muscles and connective tissue that hold your pelvic organs in place become weakened or damaged, allowing the bladder, uterus, or re**um to descend into the va**nal canal. It can happen immediately after birth or show up years later. It can feel like pressure, heaviness, a bulge you can see or feel, or just a general sense that something is not right down there.

Here is what most women are not told: surgery is not your only option. And it should not be your first one.

Pelvic floor physical therapy is the most evidence based first line of treatment for prolapse. A trained pelvic floor PT will assess the strength, coordination, and function of your pelvic floor and surrounding muscles, teach you how to manage intra abdominal pressure so you stop making it worse without knowing it, use manual therapy to address tension, scar tissue, and muscle imbalances, and build a progressive strengthening plan that actually supports your pelvic organs long term.

Surgery has its place. But going under the knife before exhausting conservative treatment is skipping the most important step.

Your body did something incredible. It deserves more than just being told to live with the aftermath.

See that dome? That is not your core working. That is showing us your 6 pack is the only thing that working. Sad, alone and all by herself.

Ideally, we have our deep core on as well, helping out your bigger abs for pressure management and stability.

And that matters way more than most people realize.

Your deep core is not your six pack. It is not the muscles you feel when you do a crunch. It is a system of four muscles working together like a canister around your spine and organs.

Your diaphragm on top. Your pelvic floor on the bottom. Your transverse abdominis wrapping around the sides. Your multifidus along your spine in the back.

When this system works the way it should, pressure gets distributed evenly. Your spine is protected. Your pelvic floor is supported. You can lift, run, sneeze, and jump without leaking, pain, or that telltale dome.

When it does not work, you get doming. Leaking. Back pain. Pelvic pressure. A core that looks strong but functionally is not.

Crunches will not fix this. Planks alone will not fix this. Learning to actually engage and coordinate your deep core will.

Your 3 pound dumbbells are not doing what you think they are doing.

If you want to actually change your body, protect your bones, balance your hormones, and feel strong well into your 50s, 60s, and beyond, you need to lift heavy. You need to rest. You need to push to failure. And you only need to do it twice a week.

Research shows that two focused strength training sessions per week is enough to build muscle, increase bone density, and improve metabolic health. More is not always better. Consistent and intentional is better.

Resting is not laziness. It is the work.
Your muscles need 2 to 3 minutes between sets to recover enough to actually perform the next set with intensity. When you rush through with 30 second breaks, you are training endurance, not strength. Rest so you can lift heavy again. That is how muscle gets built.

Progressive overload means consistently increasing the demand on your muscles over time. More weight. More reps. Less rest. Some form of progression. Without it, you plateau. With it, you keep getting stronger. No random workouts that are brand new each week.

Women lose up to 1% of bone density per year after 30. Muscle mass starts declining around the same time. Estrogen, which helps protect both, drops significantly in perimenopause and menopause.

Strength training is not vanity. It is longevity.

It protects your bones. It balances your hormones. It improves insulin sensitivity. It keeps you independent as you age. And it makes everything else in your life feel easier.

S*x should not hurt. Full stop.

Yet 3 out of 4 women will experience painful in*******se at some point in their life. And most of them will be told it is normal, to relax, to have a glass of wine - or just add more l**e!

It is not normal. And you deserve better than that answer.

Painful in*******se often comes down to your pelvic floor. When those muscles are too tight, too weak, or not coordinating the way they should, pe*******on can feel anywhere from uncomfortable to unbearable. Conditions like vaginismus, vulvodynia, and scar tissue from tearing or episiotomy are all pelvic floor issues. All treatable.

Pelvic floor PT works by addressing the actual root cause. Hands on manual therapy to release tight tissue. Scar tissue mobilization. Retraining muscles that have learned to guard and brace instead of relax.

You should not be white knuckling through intimacy. You should not be avoiding it altogether. And you should absolutely not be told this is just how it is.

Assessing the appearance of your belly during core work gives us a glimpse into what is working well and what needs some help in regards to your body.

The presence of abdominal bulging or coning as seen in the picture is generally showing us your outer abdominal muscles are taking over and doing most of the work without adequate activation of our deeper unit.

Deep core muscles do many things but generally speaking provide a corset or wrapping around to help manage the pressure. when doin its job well, the abdominal wall will be pull taut/flatter as opposed to bulging out, or tenting up.

Sometimes we can see coning when things like DRA (diastsis recti) is present.

while visually seeing bulging/coning isnt a problem by itself, its just a data point that we use along side a full strength evaluation of your core, and symptom details to curate a plan for you to squash your symptoms and strengthen your core from the inside out
