School season is upon us. Please DO NOT CALL HEALTHFIRST BLUEGRASS FOR RECORDS, THEY DO NOT HAVE THEM. I cannot guarantee any message left with the call center operators at HFBG will get to me. DO NOT MESSAGE ME ON FACEBOOK.

FAX YOUR RELEASE OF INFORMATION TO 859-447-9007. DO NOT FAX TO THE MAIN HEALTHFIRST FAX NUMBER. Make sure my name is clearly on the fax. Do NOT put "attention medical records" or it will go to medical records who may throw it out once they realize your child isn't a patient here.

I hate to have to say this over and over again. But I just got another call from the call center about records. The records are in storage and not readily accessible. They are NOT at HealthFirst.

I've deleted as many of the old posts as I can, as people are using incorrect info to request charts. Here's the process:

1. HealthFirst Bluegrass DOES NOT HAVE MY CHARTS. They are in MY possession. If you need records from care received at my office, do not ask HealthFirst for them.

2. You can mail to the old address until June 2024, after that mail will NOT be forwarded. Currently it takes at least 10 days for mail sent there to reach me. Do NOT send mail to Choice PT, they are no longer forwarding my mail to me.

3. You or your new physician can FAX a written release to 859-447-9007, or mail it to me at 496 Southland Drive, 40503. Do NOT fax to the main HFBG fax number, it is not located in my building and I cannot guarantee I will get it. If you or your provider mail a request, do NOT write "attention medical records" on the envelope, it may go to HealthFirst's medical records dept and I may never receive it. Do NOT call for records, legally I cannot release records based on a phone call and if you are not a patient at HFBG those messages may never get to me.

4. Per Kentucky statutes, one free copy of your records is provided at your request (to your new provider or to you). There is a CHARGE for subsequent copies of records.

5. Do NOT message me on this page, I do not check it regularly and have notifications turned off.

6. If your child has turned 18, only they can sign for their records.

We are open normal hours during the coronavirus crisis (8:30-4:30 Monday-Friday), and triage is still handling after hours and weekend phone calls.

If your child needs to be seen, we are spacing appointments so that only one family at a time is in our office. We are screening for possible COVID over the phone before scheduling sick visits.

We ask that, if possible, only one person accompany your child into the office.

Children scheduled for well child checkups who need vaccines are encouraged to keep that appointment. All others scheduled for routine physicals are encouraged to reschedule.
