ConnectionsPlus Healthcare + Hospice RAH’s quality of care continues to rank well above both state and national averages.

We are a community based non-profit healthcare organization specializing in supporting people with advanced illness and their families by providing palliative care, hospice and grief counseling as they navigate the winding down of this life journey. Rockbridge Area Hospice (RAH) is the only local non-profit hospice and palliative care organization serving Lexington, Buena Vista, Rockbridge County and the surrounding area. We exist as a part of our community’s healthcare system to provide knowledge, reassurance and support focused on honoring people’s wishes as they approach an unknown part of life. In fact, Medicare surveys show that that 96% of our patients’ family and friends would recommend our services to others. That is 12% higher than the national average and the highest of any hospice in our service area.

Good explanation of something many have concerns and misunderstandings about…

One of the places I meet the most resistance at the end of life is around the use of medications to relieve symptoms such as pain, agitation, or delirium. The medications I am referring to, are often morphine, lorazepam, or haloperidol, which are commonly included in the comfort or relief kit provided when someone begins hospice care. What families struggle with most is the fear that these medications may cause their person to sleep more, or become deeply sedated, and in doing so, take away the ability to have conversation.

I understand that fear. Communication at the end of life is sacred. People hold tightly to the hope of one more chance to hear their voice. I have sat at the bedside of someone I love, longing for one more word, knowing it wasn’t going to come. I struggle with this too. Wanting to keep someone as awake and cognitively present as possible comes from love.

What I have learned, though, is that when someone is dying and suffering, it can keep them from leaning into the dying process with the peace they deserve. Medications are not something I push or force. I don’t believe in taking choice away, and I never want families to carry regret or guilt into their grief. But I do believe deeply in education and support, and in helping families make peace with the fact that sometimes medication is exactly what allows suffering to soften.

What I often say in these moments is this: they might not be able to respond to you, but they can hear you. You may have already heard their last words, but they can still hear yours. I believe this with every ounce of my being. I witnessed it with my own brother who was non-responsive in the ICU. After sitting at his bedside for many days, saying all the things I had left unsaid over the years, apologizing again and again, he woke the day before he died and his last words to me were, “I’m sorry too.” He heard me.

Medications at the end of life are not what end someone’s life, their illness and the disease process do that. These medications simply allow them to die with more peace, more ease, and less suffering, and I truly believe that is something every human being deserves.

I am not here to convince you, I am here to sit with you. I want you to feel heard, and I want you to be able to make peace with a decision I know can feel heavy and complicated. I don’t want you to carry something forward that turns into an ache, wondering if you did the right thing, or if you waited too long.

My hope is to educate and support you in a way that allows you to make peace with this choice, to give medication, if it’s needed, without hesitation, and to trust that your presence matters, that your words are heard, that they are felt, and that they wrap gently around the person you love, offering comfort, safety, and permission to let go.

Be present. Speak your heart.
Let your love hold them, and let them go.
Trust that your care, your words, your presence, wraps them in the gentlest comfort. And know that in this act of love, you have done exactly what was needed.

Yesterday, Melissa Sayre—our Case Manager serving Buena Vista, Lexington, and Rockbridge—led a powerful session on Crisis Intervention for People Living with Dementia as part of the Rockbridge Area Community Services Crisis Intervention Team training.

Local law enforcement officers from the Rockbridge County Sheriff's Office, Rockbridge Regional Jail, Alleghany County Sheriff's Office and Regional Jail, and Washington and Lee University Public Safety took part. After Melissa’s presentation, officers shared personal stories and professional experiences that highlighted just how often they encounter older adults with in their daily work.

We’re grateful for the chance to partner with these dedicated teams as we work together to build safer, more compassionate, and more dementia-friendly communities.

While the officers were camera-shy, Rockbridge Area Community Services' resident therapy dog was not. 🐾🦴

Neighbors Helping Neighbors

By now, the Rockbridge Area Hospice tag-line rolls off of most people’s tongues without a second thought. But what does “neighbors helping neighbors” really mean? To our organization, it means that our team knows our community, lives in our community and loves our community. Our roots are local We are celebrating our 35th anniversary of providing end-of-life care in the Rockbridge County area. In that time span we have grown from 1 volunteer RN serving 1 patient near Goshen to a staff of 27 professionals and 55 consistent volunteers serving over 300 residents of Lexington, Buena Vista and Rockbridge County this past year. We know this area, we know the people, we know how to provide them with quality care. Our expert staff is local Over half of our direct care team hold specialty credentials in end-of-life care. Most of our staff were born here, grew up here, and/or raised families here. All of us call this area home. We live in the neighborhoods throughout the county. We relate to the families we work with, and often find we know them through some small-town connections. Our board is local Rockbridge Area Hospice is governed by a volunteer Board of Directors. These dedicated community members call this area home, and many have generational ties to Rockbridge County. The entire Board meets six times a year, and each member is engaged on one or two of several very active committees. The Board supports the organization and our staff in many ways and extends our ability to connect with our neighbors. Our response is local Having local staff means availability 24/7. Patients, families and health care providers can reach our staff any time. When an in-person visit is needed, our staff is generally able to be there within minutes, no matter what time of day or night it is. We know the back-roads and where the cell phone signals fade in and out. Our partnerships are local Our patients benefit from our partnership with local healthcare providers. We rely on Lexington Prescription Center, who provides quick home delivery of medications and equipment. We have an excellent working relationship with our local hospital, which is available to our patients for respite care or brief inpatient stays if needed. We serve many patients in local nursing homes and assisted living facilities who benefit from our support for residents who are approaching end-of-life. At Rockbridge Area Hospice, we believe that our local presence allows us to focus on quality in a way that is above and beyond typical care. When faced with situations that are a bit out of the ordinary, our discussions within our team and with our community partners focus on ‘is this the right thing to do for this person?’ If the answer is yes, we figure out how to make it happen. Our Interdisciplinary Care Teams are concentrated into a North team and a South team, each consisting of two Registered Nurses, a Care Coordinator and a Certified Nursing Aide. Other team members, including a Spiritual Counselor/Chaplain, Intake & Admissions Specialists and Bereavement Counselors, float between the two teams as specialized help is needed so that all of our patients and their families get the individualized care they deserve. This approach facilitates close communication and allows the teams to really get to know the patients and the communities they live in. In the past few years, other organizations based in Staunton or Roanoke have offered hospice services in the Rockbridge area. This gives people options to choose from. We want to be the top choice, and we are. According to Medicare’s Hospice Compare website, our “family experience of care” ranks far above other hospice organizations who come into our area, as well as the national average.
