Our phone systems are not currently working (incoming calls are not ringing). We are open 8-430 Monday through Thursday and are closed on Fridays. You can text us at 502 209 9611 during business hours and we will return your call as soon as possible. This has been a 2 plus week ordeal trying to get them fixed and we apologize for that. We will be closed the 20th of December to the 6th of January.

As a heads up for patients, our office is very busy and runs on a very small staff. Our office does not have the staff to make confirmation calls. You are expected to remember your own appointments and be at the office on time. If you miss your scheduled appointment or no show your appointment, you should expect that we will not have any available appointments for at least 2-3 months from the time you call.

We do see patients on Monday's and Tuesday's and all of us stay very busy from 8 am to 3:30 PM. If you need to call and it isn't regarding an appointment that day, we would appreciate it if you would call after 3:30 on Mon and Tues or on Wednesday or Thursday.

You must (no exceptions) wear a mask covering your nose and mouth when you are in our building. If you continually pull your mask below your nose we will ask you to leave and reschedule your appointment. Please DO NOT come to the office with out a mask!

If you are coming for an appointment you MUST have a photo ID, and your insurance card. Please bring a disc with the actual images on it of any CT's and MRI's that you have had done.

If you do not have photo ID, and insurance card (or billing information in case of an MVA) we will not be able to see you!
