Have you heard about Support Coordination?

Support Coordination at RACS begins with understanding the individual. Our team of Support Coordinators starts by assessing the needs of individuals with developmental or intellectual disabilities. Then they help to develop a plan based on each person’s strengths, goals, and areas where support is needed.

From there, one of Support Coordination’s key roles is linking individuals directly to services and providers that meet their identified needs. This connection process reduces barriers, simplifies access, and ensures individuals are connected with qualified community resources that support their success.

At its core, Support Coordination is about thoughtful planning and meaningful connections empowering individuals to access the right supports.

Cognitive and intellectual disabilities can affect how a person learns, communicates, solves problems, and processes information. Individuals with these disabilities bring unique strengths, perspectives, and talents to our communities, workplaces, and schools. With the right supports, opportunities, and inclusive environments, people with cognitive and intellectual disabilities can thrive and lead meaningful, self-directed lives.

Mental health is an important part of overall well-being, and many individuals with developmental disabilities may also experience mental health challenges. Conditions such as anxiety, depression, and other mental illnesses can affect how a person thinks, feels, and navigates daily life, but with understanding, support, and access to resources, individuals can thrive in their communities.
